摘要
目的观察并比较微等离子体射频与微针射频治疗面部萎缩性痤疮瘢痕的临床疗效及不良反应。方法对45例萎缩性痤疮瘢痕患者采用自身半侧面部对照研究,随机一侧脸使用微等离子体射频治疗,另一侧面部使用微针射频治疗,每2个月治疗1次,共3次。末次治疗后1、3、6个月进行随访,通过治疗前后痤疮瘢痕权重评分(ECCA)和治疗后不良反应评估比较两种方法的临床疗效及安全性。结果经3次治疗后,所有患者的萎缩性痤疮瘢痕均有不同程度的改善,微等离子体射频治疗侧ECCA从97.78分下降至54.11分(P<0.05);微针射频治疗侧ECCA从94.56分下降至59.00分(P<0.05)。治疗后微等离子体射频侧ECCA下降程度与微针射频侧ECCA下降程度有明显差异(P<0.001)。微等离子体射频治疗侧碾压型瘢痕平均ECCA差值与微针射频治疗侧有显著差异(P<0.05);治疗后两侧面部均有疼痛、红斑、水肿,水肿均为轻微水肿;疼痛程度、疼痛持续时间、红斑持续时间、结痂持续时间微等离子射频治疗侧均较微针射频治疗侧更强或者时间更长;微等离子体射频治疗侧治疗后有轻微渗出。两种方法治疗后均无炎症后色素沉着、感染、瘢痕加重等不良反应发生。结论微等离子体射频和微针射频对萎缩性痤疮瘢痕均有较好疗效,不良反应小,患者满意度高;微等离子体射频对萎缩性痤疮瘢痕的总体疗效优于微针射频,特别是治疗碾压型瘢痕,但微针射频不良反应相对更小,停工期更短。
Objective To observe and compare the clinical efficacy and side effects of micro-plasma radiofrequency and microneedle radiofrequency on treatment of facial atrophic acne scars.Methods Forty-five subjects with atrophic acne scar were enrolled in the study.Half of each the patient’face was randomly treated by micro-plasma radiofrequency and the other half by microneedle radiofrequency for three sessions at 2-month intervals.All patients were followed-up at 1,3,6 month after the last treatment.The clinical efficacy was evaluated by evaluation scale of acne scar(ECCA grading scale),and the side effects were analyzed by the pain scores,and the duration of pain,erythema,crusting,edema and effusion.Results There were different degrees of improvement in all subjects with atrophic acne scars after three sessions of treatment.The mean ECCA scores in micro-plasma radiofrequency group reduced from 97.78 to 54.11(P<0.05),and the mean ECCA scores in microneedle radiofrequency group reduced from 94.56 to 59.00(P<0.05).The decrement of mean ECCA score from baseline in micro-plasma radiofrequency and microneedle radiofrequency group was significantly different(P<0.001).The mean decrease in ECCA scores of rolling scars from baseline in micro-plasma radiofrequency were significantly different as comparison with microneedle radiofrequency(P<0.05).Pain,erythema,and mild edema were observed in all the patients after treated by micro-plasma radiofrequency and microneedle radiofrequency.The pain scores after micro-plasma radiofrequency treatments were more intense than after microneedle radiofrequency treatments.Crusting was observed in all patients after micro-plasma radiofrequency treatments and was observed only in ten of the nineteen patients after microneedle radiofrequency treatments.The duration of pain,erythema,and crusting of micro-plasma radiofrequency treatment group were longer than that of microneedle radiofrequency group.The effusionwas observed in all subjects only after micro-plasma radiofrequency treatments.Hyperpigm
