摘要
目的观察二氧化碳(CO_(2))激光对重度痤疮凹陷性瘢痕(AAS)的治疗效果。方法抽取2020年1月至2021年12月于洛阳市中心医院就诊的重度痤疮AAS患者98例,按随机数字表法分为对照组和观察组,每组49例。对照组行常规手术治疗,观察组行超脉冲CO_(2)点阵激光治疗,观察两组疗效及临床相关指标;采用临床痤疮瘢痕评估量表(ECCA)对治疗前及治疗后1、2、3个月瘢痕情况进行评估。治疗前及治疗3个月后,采用多功能皮肤测试仪测定患者面部皮肤生理情况。结果观察组总有效率(93.88%,46/49)高于对照组(79.59%,39/49),P<0.05;治疗后,观察组红斑持续时间、水肿消退时间、结痂持续时间均短于对照组(P均<0.05);两组ECCA评分分数在时间、组间及交互效应方面比较差异均有统计学意义(F=66.39、36.89、39.56,P均<0.05);与治疗前比较,治疗后1、2、3个月,两组ECCA评分分数均降低(P均<0.05),且观察组低于对照组(P<0.05)。治疗后,两组黑素指数、红斑指数、表皮水分子丢失情况较治疗前降低,且观察组低于对照组(P<0.05);角质层水含量较治疗前升高,且观察组高于治疗组(P<0.05)。结论CO_(2)能促进重度痤疮患者AAS的修复。
Objective To observe the effect of carbon dioxide(CO_(2))laser on severe atrophic acne scar(AAS).Methods Ninety-eight patients with severe AAS who were treated in Luoyang Central Hospital from January 2020 to December 2021 were selected,and they were divided into control group and observation group by random number table method,with 49 cases in each group.Patients in the control group were treated by conventional surgery,and patients in the observation group were treated by ultra-pulsed fractional CO_(2) laser.Curative effects and relevant clinical indicators of the two groups were observed.The Echelle d’evaluation clinique des cicatrices d’acne(ECCA)was used to evaluate the scars before treatment,1,2 and 3 month after treatment.And a multi-function skin evaluation instrument was used to determine the physiological condition of facial skin before and 3 month after treatment.Results The total effective rate in the observation group(93.88%,46/49)was higher than that in the control group(79.59%,39/49),P<0.05.After treatment,erythema duration,edema regression time,and crusting duration in the observation group were shorter than those in the control group(all P<0.05).There were statistically significant differences between the two groups in ECCA scores in terms of time effect,inter-group effect and interaction effect(F=66.39,36.89,39.56;all P<0.05).The ECCA scores of the two groups were decreased 1,2 and 3 months after treatment,compared with preoperative scores(all P<0.05),and the observation group had lower scores than the control group(P<0.05).After treatment,the melanin index,erythema index and transepidermal water loss were decreased in the two groups,and these indexes in the observation group were lower than those in the control group(P<0.05).The water content of the stratum corneum was increased after treatment,and water content of the observation group was higher than that in the control group(P<0.05).Conclusions CO_(2) laser can promote the repair of(severe)AAS.
