摘要
目的:探讨常见感染指标(PCT、超敏CRP)在脓毒症早期诊断与预后判断中的运用。方法:选取ICU收治的64例患者,其中非脓毒症患者20例,脓毒症根据2001年ACCP/SCCM共识分为:一般脓毒症15例,严重脓毒症13例,脓毒性休克16例。观察脓毒症患者1周内的病情转归,分为好转组、未愈组和死亡组。计算所有患者APACHEII评分,测定治疗1 d、3 d、5 d的PCT、超敏CRP及白细胞数据,进行相关分析。结果:与对照组比较,脓毒症患者的PCT明显升高,并随着病情的加重逐渐上升,差异有统计学意义(P<0.05)。脓毒症患者的超敏CRP、WBC比较,差异无统计学意义(P>0.05);不同转归患者PCT及超敏CRP的动态分析:PCT在好转组中有下降趋势,数值明显低于其他两组,差异有统计学意义(P<0.05);超敏CRP在好转组治疗3 d后达到高峰,治疗5 d数值降低。结论:PCT作为一项反应细菌感染的生物指标,在脓毒症中具有极高价值,有助于早期诊断和病情预后判断。
Objective To explore the application of common infection indicators(PCT and hypersensitive CRP)in early diagnosis and prognosis judgment of sepsis.Method Sixty-four patients in ICU from our hospital were selected,including 20 non-sepsis patients.According to the 2001 ACCP/SCCM consensus,sepsis can be divided into 15 cases of general sepsis,13 cases of severe sepsis and 16 cases of septic shock.To observe the prognosis of sepsis patients within a week,they were divided into improvement group,unhealed group and death group.The APACHEⅡscores of all patients were calculated,PCT,hypersensitive CRP and leucocyte data of 1,3 and 5 days after treatment were measured and analyzed.Results Compared with the control group,PCT in sepsis patients increased significantly,and gradually increased with the aggravation of the disease(P<0.05).There was no significant difference in hypersensitivity CRP and WBC between sepsis patients(P>0.05).Dynamic analysis of PCT and hypersensitive CRP in patients with different prognosis showed that PCT in the improved group had a downward trend,the value was significantly lower than that in the other two groups(P<0.05).Hypersensitive CRP peaked 3 days after treatment in the improvement group and decreased 5 days after treatment.Conclusion PCT,as a biological indicator of bacterial infection,is of great value in sepsis,which is helpful for early diagnosis and prognosis judgment.
