摘要
目的 评价并建立颈段脊髓损伤后系统化呼吸道管理及治疗模式,提高此类患者发生肺部感染治疗效果,降低肺部感染发生率.方法 观察2019年10月3日~10月31日我院重症监护室收治的颈髓损伤并发严重肺部感染患者1例,我科建立系统化呼吸道管理模式进行干预,包括高流量吸氧、机械辅助排痰、呼吸功能训练,并积极抗感染治疗及营养支持等,详实记录患者血气分析及感染指标等数据变化,以评价抗感染治疗效果.结果 临床上发生颈部脊髓损伤伴肺部感染治疗方式尚有争议,我科建立的系统化呼吸道管理与治疗模式治疗该例患者取得良好效果,可明显改善患者肺功能,提高生活质量.结论 临床上患者发生颈段脊髓损伤伴肺部感染时,应尽早建立全面完善的呼吸道管理体系并给予综合治疗,以延缓病情进展,加速康复.本文加以总结,希望为临床应用提供更多参考.
Objective To evaluate and establish a systematic respiratory management and treatment model after cervical spinal cord injury,so as to improve the therapeutic effect of lung infections in such patients and reduce the incidence rate of lung infections.Methods One patient with cervical spinal cord injury complicated with severe pulmonary infection who were admitted and treated in the intensive care unit of our hospital from October 3,2019 to October 31,2019 was observed.A systematic respiratory management model was established by our department for intervention,including high-flow oxygen inhalation,mechanically assisted sputum removal,respiratory function training,and active anti-infective treatment and nutrition support.The changes of the patient's blood gas analysis and infection indicators were recorded in detail to evaluate the therapeutic effect of anti-infective therapy.Results The clinical treatment of cervical spinal cord injury complicated with pulmonary infection is still controversial.The systematic respiratory tract management and treatment model established by our department has achieved favorable results in treating this patient,which can obviously improve the patient's lung function and quality of life.Conclusion In patients with spinal cord injury complicated with pulmonary infection,an overall and comprehensive respiratory management system should be established and comprehensive treatment should be given as soon as possible,so as to delay the disease progression and expedite recovery.This article aims to summarize the model and expects to provide more references for clinical applications.
