摘要
目的分析降钙素原(PCT)和C反应蛋白(CRP)在脓毒症患者治疗中的应用价值。方法选取2016年9月~2018年9月我院急诊监护病房(CCU)收治的98例脓毒症患者作为研究对象,根据其感染程度分为脓毒症组(n=27)、重度脓毒症组(n=48)和脓毒性休克组(n=23),选取同期收治的96例非脓毒性感染患者作为对照组。比较四组患者的PCT和CRP水平;根据28d内存活情况将98例脓毒症患者分为死亡组(n=17)和存活组(n=81),比较两组患者治疗后第1、3、7天的PCT和CRP水平。结果脓毒症组、重度脓毒症组及脓毒性休克组患者治疗前的PCT和CRP水平均显著高于对照组,差异有统计学意义(P<0.05);脓毒性休克组患者的PCT和CRP水平高于重度脓毒症组和脓毒症组,重度脓毒症组患者的PCT和CRP水平高于脓毒症组,差异均有统计学意义(P<0.05)。存活组患者治疗后第3、7天的PCT和CRP水平低于治疗后第1天,死亡组患者治疗后第3、7天的PCT和CRP水平高于治疗后第1天,差异均有统计学意义(P<0.05);存活组患者治疗后第1、3、7天的PCT和CRP水平均显著低于死亡组,差异有统计学意义(P<0.05)。结论PCT和CRP联合动态检测,有助于判断脓毒症患者病情的严重程度,对评估患者的治疗效果和预后也具有指导作用。
Objective To analyze the value of procalcitonin (PCT) and C-reactive protein (CRP) in the treatment of sepsis. Methods Ninety-eight patients with sepsis admitted to the emergency intensive care unit (CCU) of our hospital from September 2016 to September 2018 were selected as the study subjects, they were divided into sepsis group (n=27), severe sepsis group (n=48) and septic shock group (n=23) according to the degree of infection, ninety-six patients with non-septic infection were selected as the control group. The levels of PCT and CRP in four groups were compared. The 98 sepsis patients were divided into death group (n=17) and survival group (n=81) according to their 28 day survival, the levels of PCT and CRP in the first, third and seventh days after treatment were compared between the two groups. Results The levels of PCT and CRP in sepsis group, severe sepsis group and septic shock group before treatment were significantly higher than those in control group, the differences were significant (P<0.05). The levels of P CT and CRP in septic shock group were higher than those in severe sepsis group and sepsis group, the levels of PCT and CRP in severe sepsis group were higher than those in sepsis group, the differences were significant (P<0.05). The levels of PCT and CRP at 3rd and 7th day after treatment in the surviving group were lower than those in the 1st day after treatment, the levels of PCT and CRP at 3rd and 7th day after treatment in the death group were higher than those at the 1st day after treatment, the differences were statistically significant (P<0.05). The levels of PCT and CRP at 1st, 3rd and 7th day after treatment in survival group were significantly lower than those in death group on the 1st, 3rd and 7th day after treatment, the differences were statistically significant (P<0.05). Conclusion The combined dynamic detection of PCT and CRP is helpful to judge the severity of sepsis and to evaluate the therapeutic effect and prognosis of sepsis patients.
