支气管肺发育不良患儿婴儿期肺炎呼吸衰竭临床特点分析 被引量:6

Clinical characteristics of respiratory failure caused by pulmonary infection in infants with bronchopulmonary dysplasia in neonatal stage

摘要 目的探讨新生儿期发生支气管肺发育不良(bronchopulmonary dysplasia,BPD)的患儿婴儿期再次因肺部感染所致呼吸衰竭的临床特点。方法回顾性分析2013年1月至2017年6月中国医科大学附属盛京医院收治的新生儿期诊断为BPD的早产儿婴儿期(纠正胎龄<12个月)再次因肺部感染入院患儿的临床资料,根据治疗期间是否发生呼吸衰竭分为呼吸衰竭组和无呼吸衰竭组,分析BPD患儿婴儿期再次因肺部感染发生呼吸衰竭的临床特点及与新生儿状况的关系。结果符合纳入标准的BPD患儿89例,月龄(5.60±2.90)个月,男62例(69.7%);其中呼吸衰竭组54例,无呼吸衰竭组35例。呼吸衰竭组入院月龄[(5.16±2.47)个月]、体重[(4.91±1.75)kg]明显低于无呼吸衰竭组[(6.38±3.47)个月、(6.16±2.53)kg,P<0.05];中重度BPD患儿婴儿期发生肺部感染时更易发生呼吸衰竭(P<0.05)。新生儿期明确诊断为呼吸窘迫综合征的BPD患儿婴儿期发生肺炎更易发生呼吸衰竭[66.7%(36/54)比40.0%(14/35),P<0.05]。结论新生儿期BPD患儿婴儿期发生肺部感染时,年龄越小、体重越低,越容易发生呼吸衰竭。中重度BPD及发生呼吸窘迫的BPD新生儿,婴儿期发生肺部感染时更易发生呼吸衰竭。 Objective To investigate the clinical features of respiratory failure caused by pulmonary infection in infants with bronchopulmonary dysplasia(BPD)in neonatal stage.Methods We retrospectively collected the premature infants diagnosed with BPD in neonatal period(corrected for gestational age<12 months)from January 2013 to June 2017 in Shengjing Hospital of China Medical University.Respiratory failure occurred due to pulmonary infection.A total of 89 cases were divided into respiratory failure group and non-respiratory failure group according to whether respiratory failure occurred during the treatment period.The clinical characteristics of infants with BPD who suffered from respiratory infection were analyzed.Results The median age of children with BPD was(5.60±2.90)months,62 males(69.7%).Fifty-four cases were in respiratory failure group and 35 cases in non-respiratory failure group.The median age of admission[(5.16±2.47)months],average body weight[(4.91±1.75)kg]in respiratory failure group were significantly lower than those in non-respiratory failure group[(6.38±3.47)months,(6.16±2.53)kg,P<0.05].Children with moderate to severe BPD were more likely to suffer from respiratory failure when developing lung infection in infancy(P<0.05).The incidence of neonatal respiratory distress syndrome in respiratory failure group(36 cases,66.7%)was higher than that in non-respiratory failure group(14 cases,40.4%)(P<0.05).Conclusion The younger the age and the lower the weight of the infants with BPD,the more likely they are to develop respiratory failure when they suffer from lung infection in infancy.Those with moderate to severe BPD and with respiratory distress are more likely to develop respiratory failure when they have lung infection in infancy.
