摘要
目的调查分析我院静脉药物配置中心(PIVAS)抗肿瘤药物不合理医嘱,为促进临床抗肿瘤药物应用的安全性、有效性、合理性提供参考。方法对我院2015年5月至2018年5月PIVAS药师审核拦截的不合理用药医嘱进行统计分析。结果不合理医嘱共计282例,包括溶媒选择不合理130例、溶媒剂量不合理84例、给药剂量不合理19例、给药顺序不合理35例、其他14例。2015—2018年,本院PIVAS抗肿瘤药物不合理医嘱呈逐年减少趋势。结论药师通过医嘱审核、干预后,可减少或避免药品不良反应的发生,促进临床合理应用抗肿瘤药物。
Objective To investigate and analyze the unreasonable medical orders of antitumor drugs in pharmacy intravenous admixture services(PIVAS)of our hospital,so as to provide reference for promoting the safety,effectiveness and rationality of antitumor drugs application in clinic.Methods A statistical analysis was conducted on the unreasonable medical orders reviewed and intercepted by PIVAS pharmacists in our hospital from May 2015 to May 2018.Results There were 282 cases of unreasonable medical orders,including 130 cases of unreasonable solvent selection,84 cases of unreasonable solvent dosage,19 cases of unreasonable administered dose,35 cases of unreasonable dosing sequence,and 14 cases of others.From 2015 to 2018,the unreasonable medical orders of antitumor drugs of PIVAS in our hospital showed a decreasing trend year by year.Conclusion Pharmacists can reduce or avoid the occurrence of adverse drug reactions and promote the rational use of antitumor drugs in clinic after the review and intervention of medical orders.
