摘要
目的分析湖南省湘潭市中心医院静脉用药调配(静配)中心2018年不合理医嘱,提升合理用药水平。方法收集湖南省湘潭市中心医院2018年提交至静配中心的医嘱1 754 530条,对其中审方记录的不合理用药医嘱进行归纳、分析及总结。结果 1 754 530条医嘱中不合理医嘱6 068条占0. 35%,包括溶媒选择不适宜、稀释浓度不适宜、给药剂量不适宜、配伍禁忌、给药频次不适宜、给药途径不适宜等。结论经静配中心药师对医嘱进行审核,能有效减少不合理医嘱,提高临床用药安全。
Objective To analyze the unreasonable medical orders of PIVAS in Xiangtan Central Hospital,Hunan Province in 2018,and to improve the level of rational medication.Methods 1754530 medical orders submitted to PIVAS in 2018 by Xiangtan Central Hospital,Hunan Province were collected,and summarized,analyzed and summarized the unreasonable medication orders recorded.Results Of the 1754530 medical orders,6068 unreasonable medical orders accounted for 0.35%,including unsuitable choice of solvent,unsuitable dilution concentration,unsuitable dosage,incompatibility,unsuitable frequency of administration,unsuitable route of administration,etc.Conclusion Pharmacists in PIVAS reviewed the prescriptions,can effectively reduce irrational prescriptions and improve clinical safety medication.
