摘要
目的探讨磁共振成像(MRI)乳腺背景实质强化对预测新辅助化疗早期疗效的价值。方法入组我院2017年4月至2018年10月收治的乳腺癌患者100例,均接受新辅助化疗,于新辅助化疗前后进行乳腺MRI平扫和增强扫描。结果乳腺背景实质强化评估新辅助化疗疗效的准确率为87. 0%,敏感性为86. 96%,特异性为87. 10%。病理学评价为有效的患者中,新辅助化疗前后的乳腺背景实质强化的分类情况比较差异有统计学意义(P <0. 05);病理学评价为无效的患者中,新辅助化疗前后的乳腺背景实质强化的分类比较差异无统计学意义(P>0. 05)。结论 MRI乳腺背景实质强化评估新辅助化疗疗效的准确率高,可观察纤维腺体组织的强化程度,预测新辅助化疗疗效,为手术方案的制定提供数据指导。
Objective To explore the value of breast background parenchymal enhancement on magnetic resonance imaging( MRI) in predicting the early effect of neoadjuvant chemotherapy. Methods All the 100 patients with breast cancer were selected from April 2017 to October 2018 in our hospital. All the patients received neoadjuvant chemotherapy,and underwent breast MRI plain scan and enhanced scan before and after neoadjuvant chemotherapy. Results The accuracy,sensitivity and specificity of breast background parenchymal enhancement in evaluating the efficacy of neoadjuvant chemotherapy were 87. 0%,86. 96% and 87. 10%,respectively. In patients with effective pathological effect,there was significant difference in the classification of breast background parenchymal enhancement before and after neoadjuvant chemotherapy( P < 0. 05);in patients with ineffective pathological effect,there was no significant difference in the classification of breast background parenchymal enhancement before and after neoadjuvant chemotherapy( P > 0. 05). Conclusion The accuracy of breast background parenchymal enhancement on MRI is high in evaluating the efficacy of neoadjuvant chemotherapy,can observe the degree of enhancement of fibrous gland tissue and predict the efficacy of neoadjuvant chemotherapy,can provide the guidance for the formulation of surgical plan.
