摘要
目的:评价彩色超声、X线钼靶及核磁共振成像对乳腺癌诊断的价值。方法:选择2008年12月~2010年12月在天津医科大学附属肿瘤医院穿刺或手术得到病理证实的乳腺癌患者1 201例,其中1 189例行彩色超声检查,1 175例行全视野数字化X线钼靶检查,575例行核磁共振检查。结果:1 201例乳腺癌患者共检出病灶1 260个,包括双乳癌51例,单乳双灶癌8例,所有病灶中有982个经手术切除病理证实,278个病灶经粗针或细针活检证实。彩色超声、X线钼靶及核磁共振成像分别漏误诊乳腺癌182例、170例、79例。彩色超声、X线钼靶及核磁共振成像在乳腺癌诊断的准确率、敏感度、特异度方面均无明显统计学差异(P>0.05)。结论:彩色超声、X线钼靶及核磁共振成像对乳腺癌的诊断各有优缺点。在考虑经济因素的前提下,超声可以成为乳腺癌诊断首选的影像学检查方法。
Objective:To evaluate the values of ultrasonography,X-ray mammography and magnetic resonance imaging in diagnosis of breast cancer.Methods:1 201 patients with breast cancer who were diagnosed definitely by puncture,surgery and pathological examination in the hospital from December 2008 to December 2010 were selected,1 189 patients received ultrasonography,1 175 patients received whole vision digital X-ray mammography,and 575 patients received magnetic resonance imaging examination.Results:1 260 lesions were detected,51 patients were diagnosed with bilateral breast cancer,and 8 patients were diagnosed with bilateral lesions in unilateral breast,982 lesions were cut off and confirmed by pathological examination,278 lesions were confirmed by biopsy.The missed diagnosis numbers of breast cancer of ultrasonography,X-ray mammography and magnetic resonance imaging were 182,170 and 79.There was no significant difference in accuracy,sensitivity and specificity among ultrasonography,X-ray mammography and magnetic resonance imaging(P〉0.05).Conclusion:Ultrasonography,X-ray mammography and magnetic resonance imaging have respective advantages and disadvantages for diagnosis of breast cancer.Ultrasonography might be the premier choice for diagnosis of breast cancer if economic factors are considered.
