Relationship between cognitive impairment and genome-wide methylation in Alzheimer’ s disease

摘要 目的观察阿尔茨海默病(AD)患者全基因组甲基化水平的变化情况,并探讨阿尔茨海默病认知水平和全基因组甲基化的关系。方法选择山西医科大学第一临床医学院确诊的AD患者60例,并结合临床痴呆评定量表(CDR)分为轻度、中度及重度AD组,每组20例,另外选取60例健康体检老年人作为对照组。所有研究对象的认知水平采用简易精神状态量表(MMSE)和临床痴呆评定量表(CDR)进行评价,采集外周血检测全基因组甲基化率。结果AD患者全基因组甲基化率均低于对照,差异有统计学意义(P<0.05),并且随着全基因组甲基化率的降低,MMSE评分逐渐降低(r=0.493,P<0.001)。轻、中、重度AD患者全基因组甲基化率比较,中、重度AD患者的全基因甲基化降低更加明显,差异具有统计学意义(P<0.05)。结论AD患者全基因组甲基化率是降低的,并且随着认知水平的下降,全基因组甲基化率也逐渐降低,提示在临床及研究工作中,检测外周血全基因组甲基化水平很可能对AD预防、早诊断及治疗有一定临床指导价值。 Objective To observe the changes of the whole genome methylation level in patients with Alzheimer’s disease(AD),and to explore the relationship between the cognitive level of AD and the whole genome methylation.Methods Sixty AD patients diagnosed in the First Clinical Medical College of Shanxi Medical University were selected and divided into mild,moderate and severe AD groups(n=20 in each group)according to clinical dementia rating scale(CDR).Another 60 normal elderly were selected as control group.The cognitive level of all subjects was assessed by mini mental state examination(MMSE)and clinical dementia assessment scale(CDR),and the peripheral blood was collected to detect the methylation rate of the whole genome.Results The whole genome methylation rate of AD patients was lower than that of normal controls(P<0.05),and MMSE score decreased gradually with the decrease of the whole genome methylation rate(r=0.493,P<0.001).The rate of whole genome methylation was lower in patients with moderate and severe AD than that in patients with mild AD(P<0.05).Conclusion The whole genome methylation rate of AD patients is reduced,and the whole genome methylation rate is also gradually reduced with the decrease of cognitive level.It is suggested that the detection of the whole genome methylation level in peripheral blood may have a certain clinical guiding value for the prevention,early diagnosis and treatment of AD in clinical and research work.
