干扰素诱导蛋白10对早期HCV感染病情进展预测价值的研究 被引量:3

摘要 目的通过对丙型肝炎病毒(HCV)感染者干扰素诱导蛋白10(IP-10)表达水平的检测,探讨其与HCV自发清除及疾病慢性进展的相关性,观察其对HCV疾病转归的早期预测价值。方法本研究纳入66例未经抗病毒治疗的HCV感染者外周血标本,其中HCV慢性感染(HCVc)组49例,HCV自发清除(HCVr)组17例,分别对两组患者外周血抗HCV抗体,HCV RNA,IP-10,丙氨酸氨基转移酶(ALT)和天门冬氨酸氨基转移酶(AST)的表达水平进行检测。结果 IP-10水平在HCVc组的表达水平明显高于HCVr组(717.03±351.74pg/mlvs502.10±254.19pg/ml),且差异有统计学意义(P=0.024)。在HCVc组,IP-10表达水平与HCVRNA呈负相关(r=-0.394,P=0.006),而与ALT和AST均呈正相关(r=0.454和r=0.571),且差异均有统计学意义(P<0.001)。结论外周血IP-10高表达与HCV感染者疾病慢性转归有关,且IP-10与ALT和AST高表达与HCV感染初期患者特异性免疫功能较弱有关。 Objective Detect interferon-inducible protein 10(IP-10)in patients with hepatitis C virus(HCV)infection,and explore its correlation with spontaneous clearance of HCV and chronic progression of HCV infection.Methods Sixty-six untreated HCVinfected patients were enrolled in this study.Among them,49 patients with HCV chronic infection(HCVc)and 17 patients with HCV spontaneous clearance(HCVr).The expression levels of HCV antibodies,HCV RNA,IP-10,alanine aminotransferase(ALT)and aspartate aminotransferase(AST)in peripheral bloodwere tested.Results The expression level of IP-10 in the HCVc group was significantly higher than that in the HCVr group(717.03±351.74 pg/ml vs 502.10±254.19 pg/ml,P=0.024).In the HCVc group,IP-10 expression was negatively correlated with HCV RNA(r=-0.394,P=0.006),but positively correlated with ALT and AST(r=0.454 and r=0.571),and the difference was statistically significant(P<0.001).Conclusion High level of IP-10 in peripheral blood is associated with chronic HCVinfection,and during early HCV infection,the high level of IP-10,ALT and AST is associated with weakspecific immune response.
