不同血糖代谢水平机体脂肪分布比较分析 被引量:3

Comparative analysis of body fat distribution in adults at different levels of blood glucose metabolism

摘要 目的 探讨不同血糖水平成年人脂肪分布的特点。方法 回顾性选取2016年1月~2018年10月在北部战区总医院健康管理中心进行健康体检的受检者2 937例,依血糖水平分为2型糖尿病组(331例)、糖调节受损组(316例)和正常血糖组(2 290例)。测量所有受检者体质量指数(BMI)、腰臀比、体脂肪百分比、内脏脂肪面积及血脂各项指标[高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、三酰甘油(TG)]。结果 男性BMI、腰臀比、体脂肪百分比、内脏脂肪面积、LDL-C、TC、TG在糖尿病组和糖调节受损组的分布均高于正常血糖组,HDL-C在糖尿病组的分布低于正常血糖组,差异有统计学意义(P<0.05);女性各指标在3组间的分布均与男性相似,其体脂肪百分比仅在糖调节受损组高于正常血糖组,差异有统计学意义(P<0.05)。与正常血糖组相比,TG在糖尿病组较糖调节受损组增幅明显(男:32.12%vs. 15.76%,女:63.33%vs. 31.67%),男性内脏脂肪面积在糖尿病组较糖调节受损组增幅明显(11.05%vs. 7.00%),而在女性中,糖尿病组增幅却相对较低(14.55%vs. 17.25%),差异有统计学意义(P<0.05)。结论 脂代谢紊乱在糖尿病前期就已出现,除了TG增高外,还伴随有内脏脂肪面积的增高。 Objective To explore the characteristics of fat distribution in adults with different blood glucose levels.Methods 2937 physical examinees in a health management center were divided into three groups:type 2 diabetes group(331 cases),impaired glucose regulation group(316 cases)and normal glucose group(2290 cases).Body mass index,waist to hip ratio,body fat percentage,visceral fat area and various indexes of blood lipid(HDL,LDL,TC,TG)were measured.Results The male body mass index,waist to hip ratio,body fat percentage,visceral fat area,LDL,TC,TG in diabetic group and impaired glucose group were all higher than that in normal glucose group(P<0.05),and the distribution of HDL in diabetic group was lower than that in normal glucose group(P<0.05);the distribution of each index in female was similar to that in male(P<0.05),and the percentage of body fat was only in impaired glucose group,It was higher than that of normal group(P<0.05).Compared with the normal blood glucose group,TG increased significantly in the diabetic group(P<0.05);visceral fat area in the diabetic group increased significantly(P<0.05),while in the female,the increase was relatively low(P<0.05).Conclusion Lipid metabolic disorders develop in the early stage of diabetes,in addition to the increase in TG,there is also an increase in visceral fat area.
