摘要
目的探讨老年2型糖尿病血清炎性因子表达及与周围神经病变的相关性。方法选取我院2017年1月-2018年12月收治的87例老年2型糖尿病患者为糖尿病组,同期健康体检者43例为健康组;糖尿病组按照是否周围神经病变分为观察组56例、对照组31例,观察组合并周围神经病变,对照组不合并周围神经病变。检测并比较各组肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、白介素-6(IL-6)、降钙素原(PCT)等指标水平,分析各指标与周围神经病变程度相关性。结果糖尿病组TNF-α、IL-6、hs-CRP、PCT表达水平均高于健康组(P<0.05),观察组TNF-α、IL-6、hs-CRP、PCT表达水平均高于对照组(P<0.05)。临床型神经病变、临床弥散性神经病变、踝肱指数<0.5、出现糖尿病足表现的患者血清中的TNF-α、IL-6水平高于亚临床型神经病变、局灶性神经病变、踝肱指数>0.5、未出现糖尿病足表现的患者(P<0.05)。TNF-α、IL-6、hs-CRP、PCT水平随着周围神经病变程度加重而升高(P<0. 05)。结论老年2型糖尿病周围神经病变患者炎性因子水平高,且随着周围神经病变程度加重而升高,对糖尿病周围神经病变的发生发展有重大影响。
Objective To investigate correlation between expressions of serum inflammatory factors with peripheral neuropathy in elderly patients with type 2 diabetes mellitus. Methods A total of 87 elderly patients with type 2 diabetes admitted during January 2017 and December 2018 were selected as diabetes group, and other 43 healthy people taking physical examination at the same period were selected as health group. Diabetes group were divided into observation subgroup(n=56, combination of peripheral neuropathy) and control subgroup(n=31, without peripheral neuropathy) according to whether or not having peripheral neuropathy. Levels of serum tumor necrosis factor-a(TNF-α), high-sensitivity C-reactive protein(hs-CRP), interleukin-6(IL-6) and procalcitonin(PCT) were detected and compared, and correlations between each index level with degree of peripheral neuropathy were analyzed in all groups. Results Expressions of TNF-α, IL-6, hs-CRP and PCT in diabetic group were significantly higher than those in health group(P<0.05). Expressions of TNF-α, IL-6, hs-CRP and PCT in observation subgroup were significantly higher than those in control subgroup(P<0.05). TNF-α and IL-6 levels in serum of patients with clinical neuropathy, clinical disseminated neuropathy, ankle-brachial index less than 0.5 or having symptom of diabetic foot were significantly higher than those in serum of patients with subclinical neuropathy, focal neuropathy, ankle-brachial index more than 0.5 or without having symptom of diabetic foot(P<0.05). Levels of TNF-α, IL-6, hs-CRP and PCT were increased with severity of peripheral neuropathy(P<0.05). Conclusion Levels of inflammatory factors in elderly patients with type 2 diabetic peripheral neuropathy are higher than those of patients without peripheral neuropathy, and the levels may increase with severity of peripheral neuropathy, which has significant effect on development of diabetic peripheral neuropathy.
