摘要
目的:探讨妇产超声在绝经后阴道出血(VB)患者诊疗中的临床应用。方法:选取2015年3月至2018年3月西安航天总医院收治的绝经后VB患者272例,所有患者均给予妇产超声检查。结果:子宫内膜萎缩患者子宫内膜厚度明显低于子宫内膜增生症、内膜息肉、粘膜下肌瘤、内膜炎患者,子宫内膜增生症、内膜息肉、粘膜下肌瘤、内膜炎患者子宫内膜厚度明显低于子宫内膜癌,差异有统计学意义(P<0.05);以病理学为对照,超声对子宫内膜萎缩、内膜增生症、内膜息肉、粘膜下肌瘤、内膜炎、内膜癌的诊断符合率为96.88%、96.67%、100.00%、100.00%、93.33%、90.00%,超声诊断子宫内膜良恶性病变的敏感度、特异度、准确度分别为90.00%、97.62%、97.06%,差异无统计学意义(P>0.05)。结论:妇产超声对绝经后VB具有良好的诊断价值,可有效诊断患者的各类型子宫内膜病变情况,有助于判断病情进展以指导临床治疗。
Objective: To discuss the clinical application of gynecology and obstetrics ultrasound in the diagnosis and treatment of postmenopausal patients with VB. Methods: 272 postmenopausal patients with VB were selected from March 2015 to March 2018 in our Hospital, all the patients were examined by gynecology and obstetrics ultrasound. Results: The endometrial thickness in patients with endometrial atrophy was significantly lower than those in patients with endometrial hyperplasia, endometrium, submucous myoma, endomintitis, the endometrial thickness in patients with endometrial hyperplasia, endometrium, submucous myoma, endomintitis were significantly lower than that in patients with endometrial cancer, the difference was statistically significant(P <0.05). The diagnosis coincidence rate of the endometrial atrophy, endometrial hyperplasia, endometrial polyp, submucous myoma, endometritis and endometrial cancer by gynecology and obstetrics ultrasound were 96.88%, 96.67%, 100.00%, 100.00%, 93.33%, 90.00%. Specificity, accuracy of the diagnosis for benign and malignant endometrial lesions by ultrasound were 90.00%, 97.62%, 97.06%, the difference was not statistically significant(P <0.05). Conclusion: Gynecology and obstetrics ultrasound has the good diagnostic value in the diagnosis of postmenopausal VB, it can effectively diagnose the all types endometrium in patients, and it helpful to judge the progress of the disease in order to guide the clinical treatment.
