摘要
目的:探讨子宫内膜病变患者超声声像图特征及鉴别诊断分析。方法:采用数表法随机选取80例子宫内膜病变患者影像学资料,对患者超声相关表现和声像图特征进行分析,并与手术病理诊断进行对比。结果:在80例子宫内膜病变患者中,有43例为子宫内膜息肉,与手术病理诊断及结果比较相符合为41例,诊断符合率为95.35%;不符合2例。有33例为子宫黏膜下肌瘤,与手术病理诊断及结果相比较29例相符合,诊断符合率为87.88%;不符合4例。有4例为子宫内膜癌,与手术病理诊断及结果相比较4例均相符合,诊断符合率为100%。共6例误诊,其中不典型和(或)典型病例的比例为1∶16、5∶36和1∶8。各种子宫内膜病变在彩色多普勒超声图像上均有特征性的表现。结论:在对子宫腔内病变进行检查时,经阴道彩色多普勒超声检查与其他检测手段相比具有独特的声像图特征,典型疾病的声像图与手术病理标本切面相对比,两者之间存在一定程度的规律性对应关系,且诊断价值明确。
Objective:To investigate the analysis of features and differential diagnosis of ultrasonic image for patient with endometrial lesions.Methods:The imaging data of80patients with endometrial lesions were selected as random number table.The relevant ultrasound manifestations and sonographic features of patients were further analyzed,and then these data were compared with the pathological diagnosis by using operation.Results:In the80patients with endometrial lesions,43cases were endometrial polyps,and in the43cases,there were41cases were consistent with the pathological diagnosis and the diagnostic accordance rate was95.35%,while there were2cases were not consistent between the two methods.33cases of the80patients were submucous myoma of uterus,and there were29cases of them were consistent with the pathological diagnosis and the diagnostic accordance rate was87.88%,while there were4cases were not inconformity.And4cases of the80patients were endometrial carcinoma,the result was completely consistent with the pathological diagnosis and the diagnostic accordance rate was100%.In the6misdiagnose cases,the ratios of atypical cases and(or)typical cases were1:16,5:36and1:8,respectively.All of these endometrial lesions has the distinctive manifestations on the ultrasonoscopy of color Doppler.Conclusion:For the detection of endometrial lesions,transvaginal colar Doppler ultrasonic detection has unique feature of ultrasonogram compared with other detection methods.In the contrast,there is a regular congruent relationship between the ultrasonogram of typical disease and section of pathological specimen by using operation in a certain degree,and the ultrasonogram of typical disease has definite diagnosis value.
