高压氧治疗一氧化碳中毒迟发性脑病疗效的系统评价 被引量:11

Systematic evaluation of hyperbaric oxygen therapy for delayed encephalopathy caused by carbon monoxide poisoning

摘要 目的对高压氧治疗一氧化碳中毒迟发性脑病的疗效进行系统评价。方法计算机检索中国知网、万方、维普、中国生物医学文献数据库、PubMed、Embase、Cochrane Library中高压氧治疗一氧化碳中毒迟发性脑病的相关文献(检索时间为建库至2019年3月),纳入中英文随机对照试验,采用RevMan 5.2软件对数据进行meta分析。结果共纳入25篇中文文献,共1 797例患者,其中试验组(高压氧组)924例,对照组873例。与对照组相比,试验组临床有效率[相对危险度(relative risk,RR)=1.24,95%置信区间(confidence interval,CI)(1.19,1.30),P<0.000 01]、脑电图正常率[RR=2.10,95%CI(1.18,3.75),P=0.01]、简易智能精神状态检查评分[标准化均数差(standard mean difference,SMD)=3.19,95%CI(2.06,4.32),P<0.000 01]、日常生活活动能力评分[SMD=1.46,95%CI(1.02,1.90),P<0.000 01]均优于对照组。结论高压氧治疗能改善一氧化碳中毒迟发性脑病患者的症状。 Objective To systematically evaluate the effect of hyperbaric oxygen therapy on delayed encephalopathy caused by carbon monoxide poisoning.Methods China National Knowledge Infrastructure,Wanfang Data,CQVIP data,China Biology Medicine Database,PubMed,Embase,and Cochrance Library were searched by computer for randomized controlled trials on hyperbaric oxygenation for delayed encephalopathy caused by carbon monoxide poisoning which were published in English or Chinese from the dates of establishment of the databases to March 31st,2019.After literature including,excluding,and screening,RevMan 5.2 software was used to conduct a metaanalysis.Results A total of 25 studies were included,including 1 797 patients,924 in the hyperbaric oxygen therapy group(the trial group)and 873 in the control group.The clinical effective rate[relative risk(RR)=1.24,95%confidence interval(CI)(1.19,1.30),P<0.000 01],the normal rate of electroencephalogram[RR=2.10,95%CI(1.18,3.75),P=0.01],the Mini-Mental State Examination score[standard mean difference(SMD)=3.19,95%CI(2.06,4.32),P<0.000 01],and the Activities of Daily Living score[SMD=1.46,95%CI(1.02,1.90),P<0.000 01]were all higher in the trial group than those in the control group.Conclusion Hyperbaric oxygen therapy for delayed encephalopathy caused by carbon monoxide poisoning can improve symptoms.
