摘要
目的了解2016年-2017年湖州市手足口病病原学特征和流行病学情况,为手足口病的预防和控制提供科学依据。方法s用荧光定量RT- PCR方法,对695例手足口病疑似病例样本进行肠道病毒(EV)、肠道病毒71型(EV71)、柯萨奇病毒A16型( COX A16)、柯萨奇病毒A6型(COX A6)以及柯萨奇病毒A10型( COX A10)的核酸检测。结果695例标本中肠道病毒阳性率为74.39%。其中未分型肠道病毒阳性率为27.19%;CoxA16阳性率为22.73%;Cox A6阳性率为13. 81%;EV71阳性率为7. 63%;Cox A10阳性率为3.02%。不同性别男女阳性率分别为78.37%和69.21%,患者阳性率差异有统计学意义。手足口病全年均可发病,具有明显的季节性。结论2016 年一2017年湖州市手足口病的流行具有明显的双峰型,5岁以下幼儿是手足口病感染的主要人群。未分型肠道病毒和COX A16是引起儿童手足口病的主要病原,应加强手足口病病原谱变化特征的监测。
Objective To understand the etiological and epidemiological status of hand-foot-mouth disease in Huzhou City from 2016 to 2017, so as to provide scientific basis for the prevention and control of hand-foot-mouth disease. Methods Fluorogenic quantitative RT- PCR was performed on 695 suspected cases of hand,foot and mouth disease for the detection of enterovirus ( EV), enterovirus 71 (EV 71) and Coxsackievirus A16 (COX A16), Coxsackie virus A6 (COX A6) and Coxsackie virus A10 (COX A10). Results Among the 695 specimens, the positive rate was 74. 39%. The positive rate of untyped enteroviruses was 27: 19%;the positive rate of Cox A16 was 22. 73%;the positive rate of Cox A6 was 13. 81%;the positive rateof EV 71 was 7.63%,the positive rate of Cox A10 was 3.02%. There was a significant difference in the positive rate of gender between male and female, positive rates of male and female were 78. 37% and 69. 21%,respectively, and the difference was statistically significant (P< 0.05). Hand-foot-mouth disease can be onset throughout the year, with obvious seasonality. Conclusion The epidemic of hand-foot-mouth disease in Huzhou City during 2016-2017 has a clear seasonal bimodal pattern. Children under 5 years old arre the main population of HFMD infection. Untyped enteroviruses and COX A16 are the major pathogens causing hand-foot-and-mouth disease in children. Therefore, it is necessary to strengthen the monitoring of pathogenic spectrum characteristics of HFMD.
