糖尿病患者血浆与尿液白细胞介素17A水平及其与糖尿病肾病的关系 被引量:3

摘要 目的 探讨糖尿病患者血浆与尿液白细胞介素(IL)-17A的水平及其与糖尿病肾病(DN)的关系。方法 选取1型糖尿病患者(T1DM组)49例、2型糖尿病患者(T2DM组)50例及健康对照组(对照组)48例。比较3组的血脂、血清肌酐、尿白蛋白/肌酐比值(ACR)、血浆及尿液IL-17A水平。分析糖尿病患者IL-17A水平与尿ACR的相关性,并比较不同程度尿蛋白糖尿病患者血尿IL-17A水平。结果 T1DM组和T2DM组患者血清肌酐、尿ACR、血浆IL-17A及尿液IL-17A水平均高于对照组,且T1DM组血浆IL-17A水平高于T2DM组(均P<0.05)。T1DM及T2DM患者中,微量白蛋白尿组、临床白蛋白尿组血浆及尿液IL-17A水平均高于对照组,且临床白蛋白尿组尿液IL-17A水平均高于正常白蛋白尿组、微量白蛋白尿组(均P<0.05)。T1DM组与T2DM组患者尿液IL-17A水平与尿ACR均呈正相关(均P<0.05),而血浆IL-17A水平与尿ACR无明显相关性(均P>0.05)。结论 糖尿病患者血浆及尿液IL-17A水平升高,IL-17A可能参与了DN的发生发展。尿液IL-17A水平变化早于血浆IL-17A水平变化,其或可作为DN的早期检测指标。 Objective To investigate the levels of interleukin(IL)-17A in plasma and urine and its relationship with diabetic nephropathy(DN) in diabetic patients. Methods Forty-nine patients with type 1 diabetes mellitus(T1DM) were enrolled as T1DM group,50 patients with type 2 diabetes mellitus(T2DM) as T2DM group and 48 healthy controls as control group.Blood lipids,serum creatinine,urinary albumin to creatinine ratio(ACR),and plasma and urine IL-17A levels were compared among the three groups.The correlation between IL-17A level and urinary ACR was analyzed in diabetic patients,and plasma and urine IL-17A levels were compared between diabetic patients with different degrees of urinary albumin. Results Patients in the T1DM and T2DM groups had higher levels of serum creatinine,urinary ACR,plasma IL-17A and urine IL-17A levels than the control group,furthermore,plasma IL-17A level in the T1DM group was higher than that in the T2DM group(all P <0.05).In patients with T1DM or T2DM,the microalbuminuria and clinical albuminuria groups exhibited higher levels of IL-17A in plasma and urine than the control group,moreover,the clinical albuminuria group reported a higher level of urine IL-17A than the normal albuminuria group or the microalbuminuria group(all P <0.05).In patients with T1DM or T2DM,urine IL-17A level positively correlated with urinary ACR(all P <0.05),however,no significant correlation was observed between plasma IL-17A level and urinary ACR(all P >0.05). Conclusion Elevated levels of plasma and urine IL-17A are observed in diabetic patients,IL-17A may involve in the development and progression of DN.Changes in urine IL-17A level are reported prior to those in plasma IL-17A level,which may act as an early detection indicator of DN.
