摘要
目的:对应用混合多维MR成像无创评估前列腺组织成分诊断前列腺癌的可行性进行分析。方法:选取2017年1月-2018年11月的50例前列腺癌患者和50例健康体检者进行混合多维MR成像。通过拟合即可得到三区信号模型,从而计算各分区的T2值,通过成像时间、强度以及峰值对比前列腺癌患者应用混合多维MR成像参数;利用Functool软件对图像进行处理后计算患者各区带组织间质、上皮和腺泡腔的体积,同时通过Gleason评分与Spearman评分相关系数与T2值比较,进行分析。结果:前列腺癌患者与健康人群的前列腺在进行混合多维MR成像时参数、各区带组织间质、上皮和腺泡腔的体积之间比较,差异均有统计学意义(P<0.05)。其中成像时间和峰值强度之间比较,差异有统计学意义(P<0.05),前列腺癌患者上皮组织的体积分数较健康人群大,而腺泡腔与基质则健康人群大于前列腺癌患者。通过评分之间的比较可知,Gleason评分与Spearman评分相关系数与T2值比较存在显著差异(P<0.05)。结论:应用混合多维MR成像无创评估前列腺组织成分诊断前列腺癌具有可行性。
Objective:To assessment the feasibility of noninvasive estimation of prostate tissue composition by mixed multidimensional MR imaging in the diagnosis of prostate cancer.Method:50 prostatic patients and50 healthy persons from January 2017 to November 2018 were selected for mixed multidimensional MR imaging.Three-zone signal model can be obtained by fitting,and the T2 value of each zone can be calculated.Mixed multidimensional MR imaging parameters were used to compare prostate patients by imaging time,intensity and peak value.After the Functool software was used to process the images,the volumes of tissue stroma,epithelium and acinar cavity in each region of the patient were calculated,and the Gleason score and Spearman score correlation coefficient were compared with T2 value.Result:The mixed prostate of prostate patients and healthy population were mostly MR imaging parameters,and the volume of interstitial tissue,epithelium and acinar cavity in different regions were different(P<0.05).There was a significant difference between imaging time and peak intensity(P<0.05).The volume fraction of epithelial tissue in prostate patients was larger than that in healthy people,while the volume fraction of acinar cavity and matrix in healthy people was larger than that in prostate patients(P<0.05).According to the comparison between the scores,there was a significant difference between Gleason score and Spearman score in the correlation coefficient and T2 value(P<0.05).Conclusion:It is feasible to use mixed multidimensional MR imaging to noninvasively estimate the components of prostate tissue in the diagnosis of prostate cancer.
