摘要
目的探讨MR波谱成像CC/C值对中、高级别前列腺癌的诊断价值。方法回顾性分析穿刺病理活检并进行前列腺癌根治术证实为前列腺癌的患者50例,穿刺后标本进行Gleason评分。患者在进行手术及穿刺之前均行前列腺MRS检查,并分别测得每例患者感兴趣区的(胆碱+肌酸)/枸橼酸盐[(Cho+Cr)/Cit,CC/C]值。参照前列腺癌手术Gleason评分结果,将穿刺GS评分≤3+4=7患者分为中、高级别前列腺癌组(Gleason分级含5或6级)和低级别前列腺癌组(Gleason分级不含5或6级)共2组。采用独立样本t检验分析2组患者前列腺癌灶CC/C值之间的差异。结果前列腺癌患者中,中、高级别患者26例,低级别患者24例。中、高级别前列腺癌癌灶的CC/C值为2.858±1.008,低级别前列腺癌的CC/C值为0.828±0.637,差异有统计学意义(t=3.12,P=0.01)。结论对于穿刺GS评分≤3+4=7的前列腺癌患者,CC/C值有助于识别其中的中、高级别前列腺癌。
Objective To evaluate the diagnostic value of magnetic resonance spectroscopy（MRS） [（Cho ＋Cr）/Cit,CC/C] in identifying intermediate and high grade prostate cancer.Methods The MRS of 50 patients with biopsy-proven prostate cancer and biopsy Gleason score（GS） ≤3 ＋ 4=7 were retrospectively analyzed.These patients were divided into two groups,intermediate and high grade prostate cancer（patients harboring Gleason 5 and/or 6 components in their radical prostatectomy specimens） and low-grade prostate cancer（patients not harboring Gleason 5 and/or 6components in their radical prostatectomy specimens）.The CC/C values of the two groups were compared to determine whether there were differences between them with independent-sample t test（t=3.12,P=0.01）.Results Twenty-six patients had intermediate and high grade prostate cancer while the other twenty-four ones had low-grade prostate cancer.The CC/C values of intermediate and high grade prostate cancer were 2.858 ± 1.008,the CC/C values of low-grade prostate cancer were 0.828 ± 0.637.Significant differences were found for the CC/C values of these two groups.Conclusion The CC/C values of suspicious areas in prostate contribute to identify intermediate/high grade prostate cancer in patients with a GS≤3 ＋ 4=7.
