摘要
目的探讨腹腔镜联合促性腺激素释放激素激动剂(GnRH-a)治疗子宫内膜异位症(EM)的临床效果。方法选取2016年4月~2017年6月在我院住院治疗的100例EM患者,采用随机数字表法分为对照组和观察组,每组各50例。对照组实施腹腔镜治疗,观察组在腹腔镜治疗基础上给予GnRH-a治疗。比较两组术后妊娠率、治疗后血清性激素情况,治疗前后患者血清糖类抗原125(CA125)、中性粒细胞激活肽-78(ENA-78)、血管内皮生长因子(VEGF)及1年复发率。结果观察组的妊娠率为76.00%,高于对照组的40.00%,差异有统计学意义(P<0.05)。治疗后观察组CA125、ENA-78以及VEGF分别为(13.56±3.21)U/ml、(1.20±0.23)ng/ml、(82.19±2.35)pg/ml,均低于对照组的(26.24±3.67)U/ml、(2.13±0.67)ng/ml、(120.14±2.12)pg/ml,差异有统计学意义(P<0.05)。观察组1年复发率(8.00%)低于对照组(28.00%),差异有统计学意义(P<0.05)。结论腹腔镜联合GnRH-a治疗EM的效果理想,可有效改善患者的性激素水平,提高妊娠率,降低复发率。
Objective To investigate the clinical effect of laparoscopic combined with Gonadotropin-releasing Hormone Agonist(GnRH-a) in the treatment of endometriosis(EM).Methods A total of 100 patients with EM who hospitalized from April 2016 to June 2017 in our hospital were selected.They were divided into the control group and the observation group according to random number table method,50 cases in each group.The control group was treated with laparoscopic treatment,and the observation group was treated with GnRH-a on the basis of laparoscopic treatment.The pregnancy rate and serum sex hormones were compared between the two groups.Serum carbohydrate antigen 125(CA125),neutrophil activating peptide-78 (ENA-78) vascular endothelial growth factor (VEGF) and 1-year recurrence rate were measured before and after treatment.Results The pregnancy rate of the observation group was 76.00%,which was higher than that of the control group (40.00%),the difference was statistically significant (P<0.05).After treatment,the serum CA125,ENA-78 and VEGF were (13.56±3.21) U/ml,(1.20±0.23) ng/ml,(82.19±2.35) pg/ml,which lower than those of the control group for (26.24±3.67) U/ml,(2.13±0.67) ng/ml,(120.14±2.12) pg/ml,the differences were statistically significant (P<0.05).The 1-year recurrence rate (8.00%) of the observation group was lower than that of the control group (28.00%),the difference was statistically significant (P<0.05).Conclusion Laparoscopic combined with GnRH-a is effective in the treatment of EM,which can effectively improve sex hormones,improve pregnancy rate and reduce recurrence rate.
