摘要
目的探讨99mTc-DTPAGates法修正公式与99mTc-DTPAGates法、CKD-EPIcr/cysc(2012)公式评估慢性肾脏病(CKD)患者肾小球滤过率(GFR)的效果。方法将180例确诊为CKD的患者作为研究对象,行双血浆法及肾动态显影检查,分别使用99mTc-DTPAGates法和99mTc-DTPAGates法修正公式计算gGFR和pGFR;使用CKD-EPIcr/cysc(2012)公式计算eGFR。以双血浆测定法测定的tGFR值作为金标准,比较gGFR、pGFR、eGFR与tGFR之间的差异和偏离程度。结果pGFR、eGFR、gGFR和tGFR均呈正相关,相关系数分别为0.83、0.76和0.69;pGFR、eGFR、gGFR与tGFR的偏差中位数、绝对值以及精密度依次降低(P<0.05)。结论99mTc-DTPAGates法修正公式相对于CKD-EPIcr/cysc(2012)公式和99mTc-DTPAGates法用于评估CKD患者的偏差更小,准确度和精确度均较好,应在临床推广使用。
Objective To explore the effects of modified 99m Tc-DTPA Gates formula,99mTc-DTPA Gates method and CKD-EPIcr/cysc(2012)formula for evaluating glomerular filtration rate(GFR)in patients with chronic kidney disease(CKD).Methods A total of 180 patients diagnosed with CKD were used as the study subject to undergo examinations of examined by double plasma method and renal dynamic imaging.The modified formula of 99mTc-DTPA Gates method and 99mTc-DTPA Gates method were used to calculate gGFR and pGFR,and the formula of CKD-EPIcr/cysc(2012)was used to calculate eGFR.The bias,accuracy and 95%consistency of the Gates method(gGFR)and the two formulas were compared by using the tGFR values determined by double plasma assay as the gold standard.Results The positive correlation coefficients of pGFR,eGFR,gGFR with tGFR were r=0.83,0.76 and 0.69,respectively.The median bias,absolute value and accuracy of pGFR,eGFR and gGFR from tGFR decreased significantly in turn(P<0.05).Conclusions Compared with CKD-EPIcr/cysc(2012)formula and 99mTc-DTPA Gates method,the modified formula of 99mTc-DTPA Gates method has less bias,better accuracy for evaluating CKD patients,and should be popularized in clinical practice.
