分析非糖尿病慢性肾脏病1~4期患者肾小球滤过率与血浆非对称二甲基精氨酸关系的性别差异 被引量:1

摘要 目的分析非糖尿病慢性肾脏病(CKD)患者估计肾小球滤过率(e GFR)和动脉粥样硬化标志物非对称二甲基精氨酸(ADMA)是否存在相关关系及性别差异。方法选择245例诊断为非糖尿病CKD 1~4期的患者。通过横断面研究,收集血肌酐及尿素氮等有关数据,运用酶联免疫吸附法(ELISA)检测血浆ADMA。运用线性相关关系分析e GFR与血浆ADMA是否存在相关关系,探索不同性别间是否存在差异。进一步研究在不吸烟的患者中,这种相关关系是否存在性别差异。结果非糖尿病CKD 1~4期患者中,e GFR和ADMA无论在男性还是女性患者中都呈负相关关系(女性相关系数为-0.307,P<0.05;男性相关系数为-0.239,P<0.05)。但在不吸烟2组患者中,e GFR和ADMA存在明显性别差异,仅在女性患者中呈现明显负相关关系,在男性患者中未发现明显相关关系(男性相关系数r=0.126,P<0.05;女性相关系数r=0.307,P<0.05)。结论在非糖尿病慢性肾脏病患者中血浆ADMA水平与患者e GFR的下降明显相关。吸烟可能是二者存在性别差异的主要因素之一。 Objective To explore the association between estimated glomerutar filtration rate (eGFR) and Asymmetric Dimeth- ylarginine (ADMA), a marker of therosclerosis, and gender differences in patients with non -diabetic chronic kidney disease (CKD). Methods A cross - sectional study was conducted, 245 non - diabetic CKD patients stage 1 - 4 were selected. Collected the relevant clinic data such as serum creatinine, blood urea nitrogen and so on. Serum ADMA were measured by enzyme linked immunoabsordent assay (ELISA) and used linear regression to explore the association between eGFR and ADMA, and to explore the gender differences and the differences in patients without smoking. Results The patients with non -diabetic CKD stage 1 -4, eGFR and ADMA were negatively correlated in both male and female patients (r for male = - 0. 239, P 〈 0.05 ;r for female = - 0.307, P 〈 0.05). However, there were significant gender differences between eGFR and ADMA in non - smoking patients. Significant negative correlation only existed in female patients, there was no correlation in men ( r for male = 0. 126, P 〈 0.05 ; r for female = - 0. 307, P 〈 0. 05 ). Conclusion The level of ADMA is correlated to eGFR in non - diabetes CKD stage 1 - 4 patients and the gender differences may correlation with smoking.
