摘要
目的明确止血祛瘀明目片在非缺血型视网膜静脉阻塞(RVO)中的作用机制。方法选取2014年9月—2018年5月在陕西省中医院接受治疗的85例非缺血型RVO患者,对照组(40例)和治疗组(45例)。所有患者均进行常规基础治疗。对照组口服卵磷脂络合碘片。治疗组在对照组基础上口服止血祛瘀明目片。两组患者均连续治疗3个月。观察两组患者治疗前后视力、眼底出血情况,同时比较两组患者治疗前后VEGF、IL-6、IL-8和NF-κB及VCAM-1水平的变化。结果治疗组中医证候和眼底出血总有效率显著高于对照组,两组比较差异具有统计学意义(P<0.05)。治疗后两组患者视力水平较治疗前均显著提高(P<0.05),且治疗组视力水平改善程度优于对照组,两组比较差异具有统计学意义(P<0.05)。治疗后两组患者的VEGF和IL-6、IL-8水平较治疗前显著降低,两组比较差异具有统计学意义(P<0.05)。治疗后两组NF-κB及VCAM-1蛋白表达水平均较治疗前降低,但治疗组降低明显且差异有统计学意义(P<0.05)。结论止血祛瘀明目片通过下调VEGF介导的NF-κB/IL-6/IL-8/VCAM-1信号通路减轻非缺血型RVO的炎性反应进而促进眼底出血吸收,改善视功能。
OBJECTIVE To clarify the mechanism of Zhixue Quyu Mingmu Tablet in non-ischemic retinal vein occlusion.METHODS Patients(85 cases)with RVO in Shaanxi Hospital of Traditional Chinese Medicine from September 2014 to May 2018 were divided into control group(40 cases)and treatment group(45cases).All patients were given conventional basic treatment.The patients in the control group were administered with Iodized Lecithin tablet orally.The patients in the treatment group were administered with Zhixue Quyu Mingmu tablet orally on the basis of the control group.The patients in two groups were treated for 3 months.Before and after treatment,visual acuity and fundus hemorrhage were observed in the two groups,and the level of VEGF,Il-6,Il-8 NF-κB and VCAM-1 were compared between the two groups.RESULTS The total effective rate of TCM syndromes and fundus hemorrhage in the treatment group were significantly higher than counterparts in the control group,and the difference was statistically significant(P<0.05).Both groups received visual acuity improvement after treatment,and the improvement degree in the treatment group was better than that of the control group,with significant differ ence between the two groups(P<0.05).After treatment,levels of VEGF,IL-6 and IL-8 in two groups were significantly decreased(P<0.05),and the variations in two groups showed statistical difference(P<0.05).The protein expression levels of NF-κB and VCAM-1 in the two groups were reduced,but the decrease in the treatment group was more significant and the difference was statistically significant(P<0.05).CONCLUSIONS Zhixue Quyu Mingmu tablet reduced the inflammatory response of non-ischemic RVO by down-regulating the VEGF-mediated NF-κB/IL-6/IL-8/VCAM-1 signaling pathway,thereby promoting the absorption of fundus hemorrhage and improving visual function.
