摘要
本文着重于探析剔络养血法的理论内涵、遣方用药特点及临证思路。基于清代医家叶天士先生的络病理论、国医大师唐由之先生的气血辨证理论和国医大师朱良春先生的虫类药应用思想,冯俊教授结合自己30余年的临证经验总结并提出“剔络养血法”,其基本内涵是重以逐瘀剔络、兼以益气养血,从而拟定经验方——归元剔络养血方。前部缺血性视神经病变(AION)多由气血失和致使脉络壅遏受阻、血不荣络,病机本质是目络瘀阻。在病证结合模式和个体化诊疗理念指导下,应治以剔络化瘀、养血明目,并灵活运用虫类药及补气、补血之品。冯俊教授常选用归元剔络养血方为基础方,临床随症加减治疗AION,疗效颇佳。
This article emphasizes the theoretical connotation of the method of dredging collaterals and nourishing blood,the characteristics of its composition,herb selection and its clinical thinking.Based on the theory of collateral disease from Ye Tianshi,a physician in the Qing dynasty,the theory of qi and blood syndrome differentiation from Tang Youzhi,a master of Chinese medicine,as well as the thought of insect drugs application from Zhu Liangchun,a master of Chinese medicine.Professor Feng Jun combines his more than 30 years clinical experience,summarizes and proposes the method of dredging collaterals and nourishing blood.The basic connotation is to pay attention to removing stasis and dredging collaterals,supplemented by replenishing qi and nourish blood,which give birth to the empirical prescription-Guiyuan Tiluo Yangxue decoction(GTY).Anterior ischemic optic neuropathy(AION)is mostly caused by the disharmony of qi and blood,which leads to the blockage of collaterals,blood fails to nourish collaterals.Under the guidance of the combination of disease and syndrome mode and the concept of individualized diagnosis and treatment,clinical treatment is mainly based on dredging collaterals and removing stasis,nourishing blood and improving eyesight,in addition to flexible using of insect drugs and herbs of tonifying qi and blood effects.Clinically,Professor Feng Jun often uses GTY decoction as the basic prescription for the treatment of AION with modification according to specific symptoms,and receives quite favorable effects.
