急性有机磷农药中毒患者血清AST、cTnI、ChE水平变化及其与病情严重程度相关性分析 被引量:27

The Changes of Serum AST,cTnI and ChE Levels in Patients with Acute Organophosphorus Pesticide Poisoning and Their Correlations with the Severity of the Disease

摘要 目的探讨急性有机磷农药中毒(AOPP)患者血清谷草转氨酶(AST)、肌钙蛋白I(cTnI)、胆碱酯酶(ChE)水平变化及其与患者病情严重程度的关系。方法收集2013年1月至2018年1月医院收治的108例AOPP患者的临床资料,按AOPP中毒程度分为轻、中、重度组,所有患者中毒6 h内均留取外周血标本,测定血清AST、cTnI、ChE水平,分析上述指标的变化与AOPP病情严重程度的关系。结果中、重度中毒患者AST、cTnI水平高于轻度中毒患者,ChE活性低于轻度中毒患者(P<0.05),重度中毒患者AST、cTnI水平又高于中度中毒者,ChE活性低于中度中毒者(P<0. 05);中、重度中毒患者AST、cTnI、ChE异常检出率高于轻度中毒患者(P<0.05),重度中毒患者上述异常检出率又高于中度中毒者(P<0.05);中、重度中毒患者中毒指数评分高于轻度中毒患者(P <0.05),重度中毒患者中毒指数评分又高于中度中毒者(P<0.05);AST、cTnI与AOPP患者中毒指数呈正相关,ChE活性与中毒指数呈负相关(P<0.05)。结论 AOPP患者随中毒程度上升,血清AST、cTnI上升幅度增加,ChE活性受抑制程度增加,AST、cTnI与AOPP病情严重程度呈正相关,ChE与其病情严重程度呈负相关。 Objective To investigate the changes of serum aspartate aminotransferase(AST),cardiac troponin I (cTnI) and cholinesterase (ChE) levels in patients with acute organophosphorus pesticide poisoning (AOPP) and their relationships with the severity of the disease.Methods The clinical data of 108 patients with AOPP admitted to the hospital from January 2013 to January 2018 were collected.The patients were divided into the mild,moderate and severe groups,according to the severity of AOPP.Peripheral blood samples were collected from all patients within 6 hours of poisoning to determine the levels of AST,cTnI and ChE.The relationships between the changes in the above indicators and the severity of AOPP were analyzed.Results The levels of AST and cTnI showed the pattern as:patients with severe poisoning greater than patients with moderate poisoning greater than patients with mild poisoning,while the level of ChE showed the pattern as:patients with severe poisoning less than patients with moderate poisoning less than patients with mild poisoning (P <0.05).The detection rate of abnormal AST,cTnI and ChE showed the pattern as:patients with severe poisoning greater than patients with moderate poisoning greater than patients with mild poisoning (P <0.05).The poisoning index scores showed the pattern as:patients with severe poisoning greater than patients with moderate poisoning greater than patients with mild poisoning (P <0.05).The AST and cTnI levels were positively correlated with the poisoning index,while the ChE level was negatively correlated with the poisoning index of patients with AOPP (P <0.05).Conclusion With the increase of the severity of AOPP,the up-regulations of serum AST and cTnI and the suppression of ChE activity are increased.The AST and cTnI levels are positively correlated with the severity of AOPP,while the ChE level is negatively correlated with the severity of the disease.
