Fibulin-5、vWF和sCD14在急性脑梗死检测中的意义 被引量:12

摘要 目的观察扣针蛋白(Fibulin)-5、血管性血友病因子(vWF)和可溶性白细胞分化抗原14(sCD14)在急性脑梗死检测中的临床意义。方法急性脑梗死患者86例作为急性脑梗死组;选择同期在我院行健康体检患者30例作为健康对照组。运用酶联免疫反应检测2组Fibulin-5、sCD14和vWF水平。观察急性脑梗死组和健康对照组血清Fibulin-5、sCD14和vWF水平的变化,急性脑梗死患者血清Fibulin-5、sCD14和vWF水平与IMT厚度,梗死面积,神经功能损伤和预后的关系及其指标间的相互关系。结果急性脑梗死组的Fibulin-5、sCD14和vWF水平明显高于健康对照组,差异有统计学意义(P <0. 01)。急性脑梗死患者血清Fibulin-5、sCD14和vWF水平随着颈动脉IMT厚度,梗死面积和神经功能损害程度增加而升高(P <0. 01)。急性脑梗死患者预后良好组血清Fibulin-5、sCD14和vWF水平明显低于预后不良组(P <0. 01)。急性脑梗死患者血清Fibulin-5水平与sCD14和vWF水平呈正相关(r值分别为0. 816、0. 657,P <0. 05),sCD14和vWF水平呈正相关(r=0. 729,P <0. 05)。结论 Fibulin-5、sCD14和vWF参与急性脑梗死疾病的发生发展过程,对于判定急性脑梗死的严重程度和预后具有重要的临床价值。 Objective To observe the clinical significance of Fibulin-5,von Willebrand factor(vWF)and soluble leukocyte differentiation antigen 14(sCD14)in the detection of acute cerebral infarction(ACI).Methods A total of 86 patients with ACI who were treated in our hospital from January 2012 to December 2015 were enrolled as ACI group,and the other 30 subjects who received physical examination in our hospital were enrolled as control group.The levels of Fibulin-5,sCD14 and vWF were detected by enzyme-linked immunosorbent assay.The changes of serum Fibulin-5,sCD14 and vWF levels were observed in both groups,moreover,the correlation among serum levels of Fibulin-5,sCD14 and vWF in patients with ACI as well as IMT thickness,infarct size,neurological impairment and prognosis were also observed and analyzed.Results The levels of Fibulin-5,sCD14 and vWF in ACI group were significantly higher than those in control group(P<0.01).The serum levels of Fibulin-5,sCD14 and vWF in ACI group were increased with the increase of carotid IMT thickness,infarct size and neurological impairment(P<0.01).The serum levels of Fibulin-5,sCD14 and vWF in good prognosis group were significantly lower than those in poor prognosis group(P<0.01).Moreover the levels of serum Fibulin-5 in ACI group were positively correlated with sCD14(r=0.816,P<0.05)and vWF(r=0.657,P<0.05),moreover,the levels of sCD14 were positively correlated with those of vWF(r=0.729,P<0.05).Conclusion Fibulin-5,sCD14 and vWF are involved in the pathogenesis and development of acute cerebral infarction,which is of great clinical value in evaluating the severity and prognosis of acute cerebral infarction.
