脑梗死血清MMP-3与MMP-9的动态变化及临床意义 被引量:12

摘要 目的:观察血清基质金属蛋白酶3(MMP-3)与基质金属蛋白酶9(MMP-9)在脑梗死患者不同时期的动态变化,探讨其临床意义。方法:62例脑梗死和腔隙性脑梗死患者分别分为观察组1和2,30例健康体检者为对照组,分别检测观察组发病2-5 d、15 d及对照组的血清MMP-3、MMP-9的含量及神经功能缺损程度评分(CNFDS)。结果:血清MMP-9和MMP-3水平发病2-5 d时,观察组1、2与对照组比较均显著升高(P<0.01),观察组1 MMP-9水平比观察组2升高更显著(P<0.01);发病15 d时,观察组1、2 MMP-9和MMP-3水平与对照组比较差异无显著性意义。发病2-5 d的血清MMP-9和MMP-3水平与预后呈负相关。结论:脑梗死发病早期血清MMP-9与MMP-3水平的高低与临床病程有关,而且是判断脑梗死近期预后的重要指标。 Objective: To investigate the dynamic changes and clinical significance of serum matrix metalloproteinase-3 (MMP-3) and matrix metalloproteinase-9 (MMP-9) on various stages in patients with cerebral infarction. Methods: 62 cerebral infarction and lacunar infarction patients were divided into the observation group one and the observation group two, while 30 healthy people served as the control group. Serum MMP-3 and MMP-9 levels and the scores of the Clinical Neurological Function Deficient (CNFDS) were measured during the onset 2 to 5 days and 15 days in the observation group and the control group. Results: The levels of serum MMP-9 and MMP-3 during 2 to 5 days of the cerebral infarction were higher than those in the control group (P〈0.01). The serum MMP- 9 levels in the observation group one were significantly higher than those in the observation group two (P^0.01). The levels of serum MMP-3 and MMP-9 at the onset 15 days of the observation groups had no significant difference compared with those of the control group. Correlation analysis showed that there was a negative correlation between MMP-9, MMP-3 and prognosis. Conclusion: MMP-3 and MMP-9 may contribute to the pathologic course in the early stage of cerebral infarction. The levels of MMP-3 and MMP-9 are correlated with the clinical course of the disease and prognosis.
