摘要
目的探讨自制一体化冲洗引流管治疗肱骨骨折钢板内固定术后感染的疗效。方法 2011-10~2016-10,应用自制一体化冲洗引流管治疗肱骨骨折钢板内固定术后感染的患者23例,其中男16例,女7例,年龄18~62岁,平均年龄38.2岁。骨折部位:肱骨外科颈骨折7例,肱骨干骨折13例,肱骨髁上骨折3例。所有病例均拆除钢板更换单臂外固定架或石膏外固定,彻底清创后持续冲洗引流。观察自制一体化冲洗引流管的冲洗效果和伤口愈合情况。结果所有患者按照疗效判断标准随访1~2年,痊愈19例,显效1例,有效1例,无效2例,总有效率为91.3%。结论取出内固定物并彻底清创,利用自制一体化冲洗引流管持续冲洗引流是治疗肱骨骨折钢板内固定术后感染的有效方法。
Objective To explore the curative effect of self-made integrated irrigation and drainage tube on treatment of postoperative infection of humeral fractures.Methods A retrospective analysis was performed on 23 patients who received the lavage therapy with self-made irrigation and drainage tubes during October 2011 and October 2016,among whom there were 16 males and 7 females,aged 18~62 years(a mean of 38.2 years).The fracture sites included surgical neck of humerus in 7 cases,humeral shaft in 13 cases and humeral supracondylar in 3 cases.All of the cases were dislodged or replaced fixation after debridement,then irrigated to a great extent.The irrigation results and wound healing were observed during treatment.Results After a follow-up of 1 to 2 years,19 cases were cured,1 markedly effective,1 effective and 2 ineffective,with an effective rate reaching 91.3%.Conclusion Removal of the internal fixator and thorough debridement,continuous irrigation and suction are effective measures to control infections after internal fixation of humeral fracture.
