摘要
目的分析肱骨外科颈骨折术后肩关节功能障碍的原因。方法选取该院2018年1月—2021年7月收治的86例肱骨外科颈骨折术后患者作为该次研究对象,分为对照组(n=70,术后无肩关节功能障碍)及研究组(n=16,术后肩关节功能障碍),各16例,并对肩关节功能障碍因素进行调查。结果年龄、治疗方式、骨质疏松症、肩袖损伤以及骨折类型是肱骨外科颈骨折术后肩关节功能障碍的相关因素(P<0.05),对照组50岁以上占比为84.85%、治疗方式中保守治疗占比为92.11%、有骨质疏松症占比为64.29%、有肩袖损伤占比为92.86%,骨折类型中肱骨外科颈骨折合并肩节前脱位占比为96.15%,与研究组比较,差异有统计学意义(Z=7.631,χ^(2)=5.157、5.360、7.122、18.918,P<0.05)。多因素Logistic回归分析表明:在影响肱骨外科颈骨折术后患者肩关节功能障碍因素中发现,在患者年龄、骨质疏松症、肩袖损伤、治疗方式、骨折类型等方面和肩关节功能障碍呈现紧密的关系(P<0.05)。结论针对肱骨外科颈骨折术后患者进行合理干预,可以有效预防肩关节功能障碍的发生,为后续临床治疗提供重要的科学依据,值得临床推广应用。
Objective To analyze the causes of shoulder joint dysfunction after surgical neck fracture of humerus.Methods 86 patients with humeral surgical neck fracture treated in the hospital from January 2018 to July 2021 were selected as the study subjects.They were divided into the control group(n=70,without shoulder dysfunction after operation)and the study group(n=16,with shoulder dysfunction after operation).The factors of shoulder dysfunction were investigated.Results Age,treatment method,osteoporosis,rotator cuff injury and fracture type were the related factors of shoulder joint dysfunction after surgical neck fracture of humerus(P<0.05).In the control group,the proportion of patients over 50 years old was 84.85%,the proportion of conservative treatment was 92.11%,the proportion of patients with osteoporosis was 64.29%,the proportion of patients with rotator cuff injury was 92.86%,and the proportion of fracture type humeral surgical neck fracture combined with anterior shoulder dislocation was 96.15%,Compared with the study group,and the difference was statistically significant(Z=7.631,χ^(2)=5.157,5.360,7.122,18.918,P<0.05).Multivariate logistic regression analysis showed that there was a close relationship between the shoulder dysfunction and patients'age,osteoporosis,rotator cuff injury,treatment method and fracture type among the factors affecting shoulder dysfunction after surgical neck fracture of humerus(P<0.05).Conclusion Reasonable interventions for patients with shoulder joint dysfunction can effectively prevent the occurrence of shoulder joint dysfunction,provide important scientific basis for subsequent clinical treatment,and are worthy of clinical application.
