CT多平面重建在股骨颈骨折临床分型评估中的应用 被引量:11

摘要 目的:探讨螺旋CT多平面重建(MPR)在股骨颈骨折(FNF)临床分型的应用价值。方法:选取120例FNF患者为研究对象,对其X线检查及CT的MPR影像学资料进行回顾性分析。以手术病理分型为“金标准”,比较X线检查及CT的MPR对FNF不同骨折类型的检出率。结果:螺旋CT的MPR对FNF解剖分型、Garden分型、Pauwels分型及AO分型的诊断符合率分别为96.67%(116/120)、95.00%(114/120)、93.33%(112/120)、95.00%(114/120),与手术病理诊断结果相近,且高于X线检查的83.33%(100/120)、81.67%(98/120)、83.33%(100/120)、85.00%(102/120)(P<0.05)。结论:螺旋CT的MPR可有效鉴别FNF的解剖分型、Garden分型、Pauwels分型及AO分型,为临床诊治FNF提供影像学依据,且较X线检查有更大的优势。 Objective:To explore the value of spiral CT multiplanar reconstructions(MPR) in clinical classification of femoral neck fractures(FNF).Methods:The data of 120 patients with FNF were collected.X-ray examination and CT MPR data of the patients were retrospectively analyzed.Surgical and pathological typing was taken as the golden standard, the detection rates of different fracture types of FNF by MPR of X-ray examination and CT were calculated and compared.Results:The diagnostic coincidence rates of spiral CT MPR for anatomical classification, Garden classification, Pauwels classification and AO classification of FNF were 96.67%(116/120),95.00%(114/120),93.33%(112/120) and 95.00%(114/120),respectively.The results were similar to surgical and pathological results.In addition, the coincidence rates were higher than those of X-ray examination [83.33%(100/120),81.67%(98/120),83.33%(100/120),85.00%(102/120),P<0.05].Conclusion:Spiral CT MPR can effectively identify the anatomical type, Garden type, Pauwels type and AO type of FNF,which provides an imaging basis for clinical diagnosis and treatment of FNF.Besides, it is superior to X-ray examination.
