摘要
目的探讨万古霉素骨水泥用于骨折内固定术后髓内感染的临床价值。方法回顾性分析2013年1月至2014年6月通海骨伤医院收治的28例应用万古霉素骨水泥治疗骨折术后髓内感染患者的临床资料;彻底清创扩髓后植入万古霉素骨水泥;术后按药敏结果给予抗菌药物治疗,记录患者感染控制时间及复发情况。结果所有患者均获得随访,平均随访时间14个月。其中26例患者感染均得到控制,治疗时间为3-6个月,平均治疗时间为（4.1±0.7）个月。1例因局部反复有渗出,感染未控制,1例患者感染复发,均经再次清创万古霉素骨水泥置入,感染得到控制;按临床疗效结果判定,本研究优23例,良3例,差2例,优良率为92.9%。结论采用万古霉素骨水泥填塞法治疗骨折内固定术后髓内感染安全有效,可减少感染复发,临床价值高值得推广。
Objective To discuss the clinical value of vancomycin-loaded bone cement for the treatment of intramedullary infection after internal fixation.Methods The clinical data of 28 patients with intramedullary infection admitted in our hospital from January 2013 to June 2014 were retrospectively analyzed.All the patients were given complete debridement,medullary expanding and vancomycin bone cement implant,as well as antibiotics according to antimicrobial susceptibility results.The infection control time and recurrence of the patients were recorded.Results All the patients were followed up and the average follow-up time was 14 months.26patients had their infection well controlled;the treatment time was 3to 6months,（4.1±0.7）months on average.1case had recurrent local exudate,whose infection became out of control,and 1case had recurrence of infection.Both patients were given debridement and vancomycin bone cement placement again,and their infection became under control.The evaluation of treatment reveal that there were 23 cases with optimal of clinical efficacy and 3with fine efficacy,while 2with poor efficacy cases,with an efficiency rate of 92.9%.Conclusion Vancomycin-loaded bone cement is safe and effective for treatment of intramedullary infection after internal fixation,which can reduce the recurrence of infections and is of high clinical value.
