摘要
目的比较广西某三级医院外埠患者疾病构成和费用,分析其现状和影响因素。方法选择某院2013年-2017年外埠患者的病例资料,比较4种不同来源地的住院费用构成及疾病构成。结果外埠患者由7684人增至8002人,人均费用由10 450元增至11 736元。不同范围的外埠患者的住院费用构成有显著性差异,药品费占住院费用比重最大约30%。不同范围的前8病种人均费用有显著性差异,分级诊疗前后外埠患者年均住院例数无明显变化。疑难重症患者占外埠患者逐渐升高。结论建立区域性医联体和实行分级医疗制度,控制药品、医用耗材和大型医疗设备检查费用是控制医疗费用增长的有效措施。
Objectives To compare the disease compositions and expenses of outbound patients in a tertiary hospital of Guangxi in the past 5 years and analyze its current situation and the influencing factors. Methods Selected the outbound patients in a hospital to compare the disease compositions and expenses of 4 different sources from 2013 to 2017. Results The number of patients outside the port increased from 7,684 to 8,002,and the average cost increased from 10,450 to 11,736 yuan from 2013 to 2017. There was significant difference in the cost of hospitalization between patients in different areas.Drugs accounted for the largest share of hospitalization costs（about 30%）. There was significant difference in the per capita cost of the first 8 diseases.There was no significant change in the annual number of hospital admissions for patients outside the port before and after the level diagnosis and treatment. The proportion of the patients with difficult and severe diseases in foreign towns were increasing gradually. Conclusions The other costs of non-surgical materials were highest in Guangdong,while the other costs of non-surgical materials were highest in both areas. The establishment of regional medical association and the implementation of a hierarchical medical system,control of drugs,medical consumables and large medical equipment inspection fees were effective measures to control the increase of medical costs.
