摘要
目的:观察炎调Ⅱ号方对脓毒症大鼠凝血功能的影响,并探讨其作用机制。方法:清洁级健康雄性SD大鼠随机分为正常组、假手术组、模型组、炎调Ⅱ号方组及四妙勇安汤组。正常组以0.9%NaCl溶液灌胃1 d,不手术;假手术组以0.9%NaCl溶液灌胃1 d后行假手术(经麻醉后开腹,轻轻翻动肠道后关腹);炎调Ⅱ号方组、四妙勇安汤组、模型组分别以炎调Ⅱ号方(9.9 g/kg)、四妙勇安汤(9.9 g/kg)及等容量0.9%NaCl注射液灌胃1 d后采用盲肠结扎穿孔术(CLP)制备脓毒症模型。分别于造模后12 h处死动物,并进行肺组织HE染色,采用ELISA法测定大鼠凝血功能及血清组织因子(TF)、组织因子抑制剂(TFPI)、血管性血友病因子(vWF)、血栓调节蛋白(TM)、可溶性血管内皮细胞蛋白c受体(SEPCR)含量。结果:肺组织HE染色结果显示,模型组可见肺组织毛细血管充血,间质增生,黏液分泌堵塞,肺组织破坏;炎调Ⅱ号方及四妙勇安汤组肺组织损伤范围较小,肺泡量较多,肺内微血栓少,肺间质水肿、炎性细胞浸润及肺实变较模型组减轻。模型组大鼠血活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、国际标准化比率(INR)、血浆凝血酶原时间(PT)值较假手术组明显升高(P<0.05);炎调Ⅱ号方组、四妙勇安汤组APTT、FIB、INR、PT值较模型组明显降低(P<0.05)。模型组大鼠TF、TFPI、vWF、TM、SEPCR水平较假手术组显著增高(P<0.05);炎调Ⅱ号方组、四妙勇安汤组大鼠TF、TFPI、vWF、TM、SEPCR水平较模型组显著降低(P<0.05)。炎调Ⅱ号方组TF、TM水平较四妙勇安汤组下降明显,差异有统计学意义(P<0.05)。结论:炎调Ⅱ号方可能通过干预血管内皮细胞功能改善脓毒症大鼠凝血功能障碍,从而有效减轻脓毒症大鼠肺内微血栓形成,改善肺组织的损伤和实变程度。
Objective: To observe the effects of Yantiao No. Ⅱ Formula on coagulation function of rats with sepsis,and explore its mechanism. Methods: The healthy male SD rats of clean grade were randomly divided into the normal group,sham operation group,model group,Yantiao No.Ⅱ Formula group and Simiao Yong'an Decoction group. The normal group was treated with 0. 9% NaCl solution by intragastric administration for 1 day,with no operation. The sham operation group was performed the sham operation after intragastric administration of 0.9% NaCl solution for 1 day. The abdominal cavity of rats in the sham operation group was opened after anesthesia,and then closed after gently turned the intestines. The model group,Yantiao No.Ⅱ Formula group and Simiao Yong'an Decoction group were respectively treated with 0. 9% NaCl solution( equal volume),Yantiao No. Ⅱ Formula( 9. 9 g/kg) and Simiao Yong 'an Decoction( 9.9 g/kg) by intragastric administration for 1 day. And then the sepsis model was prepared by cecal ligation and puncture( CLP). 12 hours after modeling,the animals were sacrificed,the lung tissue was stained by hematoxylin and eosin( HE),the coagulation function and serum levels of tissue factor( TF),tissue factor pathway inhibitor( TFPI),von willebrand factor( vWF),thrombomodulin( TM),soluble endothelial protein C receptor( SEPCR) were detected by ELISA. Results: The HE staining of lung tissue showed that the pulmonary capillary hyperemia,interstitial hyperplasia,mucus secretion blockage and lung tissue damage were observed in the model group. While the lung tissue damage scope was smaller,the alveolar volume was bigger and the pulmonary microthrombus was fewer in the Yantiao No.Ⅱ Formula group and the Simiao Yong'an Decoction group,and the pulmonary interstitial edema,inflammatory cell infiltration and pulmonary consolidation were alleviated compared with the model group.
