摘要
目的探讨中性粒细胞/淋巴细胞比值(NLR)及感染性指标在肺结核早期诊断中的应用价值。方法收集2016年1月-12月入院的肺结核患者192例,在患者入院24 h内采集EDTA抗凝血,分别做外周血细胞计数、淋巴细胞亚群检测及SAA等感染性指标检测。结果肺结核组NE、NLR、RDW、SAA、CRP、CD4、CD4/CD8值均升高,而LY值降低,与健康对照组相比差异均有统计学意义(P<0.05);外周血白细胞、淋巴细胞、中性粒细胞、中性粒细胞/淋巴细胞比值、RDW的曲线下面积分别为0.579、0.731、0.165、0.863、0.874;感染性指标SAA、CRP、SAA/CRP、PCT的曲线下面积分别为0.484、0.450、0.501、0.365。结论 NLR、淋巴细胞作为感染的生物标志物,优于其他传统感染性指标;在肺结核患者早期诊断中有重要的应用价值,并与疾病的严重程度和预后密切相关。
Objective To investigate the application value of NLR and SAA in the early diagnosis of pulmonary tuberculosis. Methods 192 cases of pulmonary tuberculosis in our hospital were collected in this study from January to December in 2015. EDTA anticoagulation was collected within 24 hours after admission, and peripheral blood cell count, lymphocyte subset and SAA were detected. Results The levels of NE, NLR, RDW, SAA, CRP, CIM and CIM/CD8 in the tuberculosis group increased, while the LY value decreased, and the differences were statistically significant compared with that in the control group （P〈0.05）. The area under the curve of peripheral blood leukocytes, lymphocytes cells, neutrophils, neutrophils/lymphocytes ratio, and RDW was O.579, O.731, 0.165, 0.863, and 0.874, respectively. The areas under the curve of infectious index SAA, CRP, SAA/CRP, and PCT were 0.484, 0.450, 0.501, 0. 365. Conclusion As the infection biomarkers, NLR and lymphocytes are superior to other traditional infectious indicators, which have important application value in the early diagnosis of pulmonary tuberculosis and are closely related to the severity and prognosis of the disease.
