摘要
[目的]分析血清胆碱酯酶(CHE)对细菌性肺部感染成年患者炎症反应及免疫水平的影响。[方法]选取2017年2月至2020年2月在某医院接受治疗的肺炎患者50例为观察组(年龄28~59岁),选取同一时期在该院参与体检的健康体检者50例为对照组(年龄27~57岁)。两组一般基础资料差异无统计学意义(P>0.05)。观察组患者于入院后第2天8时前,空腹,通过肘静脉采血5 mL,并在采血后30 min内分离血清(3 500 r/min,离心10 min),采用丁酰硫代胆碱法检测CHE。另采集5 mL静脉血,用于常规的血常规、C反应蛋白(CRP)检验等检测。对照组在体检时通过肘静脉采血,并采用上述方法进行检测。[结果]与对照组比较观察组的CHE明显降低,并伴有CRP、PCT、WBC、中性粒细胞升高,差异均有统计学意义(P<0.05)。观察组的T细胞亚群检测结果中[CD4]及[CD4]/[CD8]均低于对照组,差异有统计学意义(P<0.05)。观察组CHE与各指标(CRP、PCT、WBC、中性粒细胞、[CD4]/[CD8])之间的相关性分析,CHE与CRP、PCT、WBC、中性粒细胞显著呈负相关,差异有统计学意义(P<0.05),与[CD4]/[CD8]呈正相关差异有统计学意义(P<0.05)。[结论]血清胆碱酯酶(CHE)降低与细菌性肺部感染成年患者的炎症反应及免疫水平降低有一定的影响。
[Objective]To analyze the serum cholinesterase(CHE)of bacterial lung infection in adult patients with inflammation and immunity.[Methods]The influence of selected from February 2017 to February 2020 in our hospital for treatment of 50 cases of pneumonia patients.As the observation group,the age(28~59)was selected as the control group for 50 healthy subjects who had participated in the physical examination in the same period.Age(27~57)years old.There was no statistically significant difference between the two groups(P>0.05).The serum was isolated within 30 minutes after the venous blood was collected(3500 r/min,and the centrifugal time was 10 min).CHE using butyiyl glucosinolates bile alkaline or testing,the other 5 mL venous blood collected for regular-blood tests,the control group in physical examination by elbow vein venous blood was collected,through the above methods for testing.[Results]Two groups of CHE comparative observation group is higher than the control group and observation group of serum cholinesterase(CHE)decreased obviously,and associated with higher-CRP and PCT,the WBC,neutrophil,differences were statistically significant(P<0.05).Observing group of testing results of T cell subgroup[CD4]and[CD4]/[CD8]are lower than the control group,the difference was statistically significant(P<0.05),the observation group of serum cholinesterase(CHE)and the index(CRP and PCT,the WBC,neutrophil,[CD4]/[CD8])correlation between serum cholinesterase(CHE)and CRP and PCT,the WBC,neutrophil significantly negatively related to the difference was statistically significant(P<0.01),and[CD4]/[CD8]were positively correlated difference was statistically significant(P<0.01).[Conclusion]serum cholinesterase(CHE)decreased the inflammatory response and decreased immune level in adult patients with bacterial lung infection.
