摘要
目的探讨检测支气管肺泡灌洗液中miR-205和miR-21用于肺癌诊断的临床价值。方法采用荧光定量PCR法检测22例肺癌患者和26例良性肺疾病患者支气管肺泡灌洗液miR-205和miR-21的表达水平。分析其与患者临床病理特征的相关性。构建受试者工作特征(ROC)曲线,探讨肺泡灌洗液中miR-205和miR-21诊断肺癌的效能。结果肺癌患者支气管肺泡灌洗液miR-205和miR-21的表达水平均明显高于良性肺疾病对照组,差异有统计学意义(P<0.01)。支气管肺泡灌洗液miR-21表达与患者的临床病理特征无相关性,而miR-205表达水平与肺癌的组织学分型显著相关(P<0.01),与肺癌患者的性别、年龄、吸烟与否、分化程度及临床分期不相关。支气管肺泡灌洗液miR-205和miR-21的ROC曲线下面积(AUC)分别为0.748(95%CI 0.598~0.899)和0.797(95%CI 0.673~0.922),两者联合检测的ROC曲线下面积为0.830(95%CI 0.709~0.952),区别肺癌患者和良性肺疾病患者的敏感度为81.8%,特异度为76.9%。结论检测BALF中miR-205和miR-21的表达对肺癌具有一定的诊断价值,支气管肺泡灌洗液中miR-205和miR-21有可能作为诊断肺癌的生物标志物。
Objective To investigate the clinical value of detecting miR-205 and miR-21 in bronchoalveolar lavage fluid(BALF) for the diagnosis of lung cancer. Methods The levels of BALF miR-205 and miR-21 in 22 patients with lung cancer and 26 patients with benign lung diseases were detected by fluorescence quantitative PCR. The correlation between the two miRNAs levels and clinicopathological characteristics in patients with lung cancer was analyzed. The receiver operating characteristic(ROC) curve was constructed to investigate the efficacy of BALF miR-205 and miR-21 in the diagnosis of lung cancer. Results The levels of BALF miR-205 and miR-21 in patients with lung cancer were significantly higher than those of benign lung diseases control group,with the differences statistically significant(P〈0. 01). The expression of miR-21 in bronchoalveolar lavage fluid was not correlated with the clinicopathological characteristics of patients,but the expression of miR-205 was significantly correlated with the histological type of lung cancer(P〈0. 01),but not with gender,age,smoking,differential degree and clinical stages. The area under the ROC curve(AUC) of BALF miR-205 and miR-21 were 0. 748(95% CI: 0. 598-0. 899) and 0. 797(95% CI: 0. 673-0. 922),respectively. The area under ROC curves of combined detection of two indicators was 0. 830(95% CI: 0. 709-0. 952),and the sensitivity and specificity were respectively 81. 8%and 76. 9%. Conclusion The levels of miR-205 and miR-21 in BALF had a great clinical value in lung cancer diagnosis and may be used as promising biomarkers of lung cancer.
