血清IL-21、EOS、miR-21、PCT对急性加重期COPD百令胶囊治疗的疗效评估价值

摘要 目的探讨血清白介素-21(IL-21)、嗜酸性粒细胞(EOS)、微小RNA-21(miR-21)、降钙素原(PCT)对急性加重期慢性阻塞性肺疾病(COPD)百令胶囊治疗的疗效评估价值。方法选取我院2019年7月至2020年9月收治的120例急性加重期COPD患者,均使用百令胶囊治疗。比较有效组和无效组的临床资料及血清学指标,Logistic回归分析影响治疗效果的因素,分析血清IL-21、EOS、miR-21、PCT单独及联合检测预测治疗效果的价值并编制ROC曲线。结果治疗12周后,显效60例,有效38例,无效22例,总有效率为81.67%。有效组血清IL-21、EOS、miR-21、PCT低于无效组(P<0.05);血清IL-21、EOS、miR-21、PCT均是影响急性加重期COPD患者治疗效果的因素(P<0.05);血清IL-21、EOS、miR-21、PCT联合检测预测治疗效果的AUC值高于单项检测(P<0.05)。结论血清IL-21、EOS、miR-21、PCT对急性加重期COPD百令胶囊治疗的疗效具有一定预测价值。 Objective To evaluate the efficacy of serum interleukin-21(IL-21),eosinophils(EOS),micrornA-21(miR-21)and procalcitonin(PCT)in the treatment of acute exacerbation chronic obstructive pulmonary disease(COPD)with Bailing capsule.Methods A total of 120 patients with acute exacerbating COPD admitted to our hospital from July,2019 to September,2020 were selected and treated with Bailing capsule.The clinical data and serological indexes of the effective group and the ineffective group were compared.Factors influencing the therapeutic effect were analyzed by logistic regression,and the value of serum IL-21,EOS,miR-21 or PCT only and combined detection to predict the therapeutic effect were analyzed.ROC curve was drawn for evaluation.Results After 12 weeks of treatment,60 cases were strongly effective,38 cases were effective,22 cases were ineffective,and the total effective rate was 81.67%.Serum IL-21,EOS,miR-21 and PCT in the effective group were lower than those in the ineffective group(P<0.05).Serum IL-21,EOS,miR-21 and PCT were all factors affecting the therapeutic effect of patients with acute exacerbation of COPD(P<0.05).AUC value of serum IL-21,EOS,miR-21 and PCT combined detection was higher than that of single detection(P<0.05).Conclusion Serum IL-21,EOS,miR-21 and PCT have a certain predictive value for the therapeutic effect of Bailing capsule in acute exacerbation of COPD.
