摘要
目的了解有婚史的男男性行为者（MSM）的艾滋病病毒（HIV）感染状况及其行为心理特征,为开展针对性干预提供建议。方法通过志愿者在重庆主城区MSM活动场所和有关QQ群,以滚雪球方式招募调查对象,由经统一培训的调查员进行面对面的问卷调查和HIV检测,用SPSS 22.0进行统计分析。结果共调查330例有婚史的MSM,新确认HIV阳性27人（8.2%）。调查对象平均年龄（45.4±6.9）岁,85.5%的调查对象有子女。在婚状态的调查对象79.1%与妻子及子女住在一起。调查对象首个性伴为女性的占69.4%。95.1%的调查对象与男性发生肛交,最近一年安全套坚持使用率为53.8%;11.5%曾参与群交。119人（36.1%）最近一年和妻子有性生活,其中31.1%每月至少1次性行为,安全套坚持使用率为31.9%。55.6%的人认为同性性行为对婚姻及家庭生活无影响,仅57人（17.3%）的妻子知晓自己性取向状态,对目前感情生活状态感觉非常压抑的占24.2%。HIV感染多因素Logistic回归分析发现,艾滋病知识知晓情况差[比值比（OR）=7.047,95%可信区间（CI）：1.626~30.542]、使用精神活性物质（OR=2.881,95%CI：1.129~7.348）、肛交时安全套使用率低（OR=4.315,95%CI：1.920~9.695）者HIV感染的风险更高。结论有婚史的MSM是HIV高感染率群体之一,因其与同性和异性间都存在高危性行为,在不同群体间造成HIV交叉感染,应进一步探索针对该亚人群的行为干预和心理疏导措施。
Objective To explore the prevalence of HIV and its associated behavior and psychological characteristics among married men who have sex with men（MSM）in Chongqing.Methods With support of MSM community-based groups,330 married MSM were enrolled for questionnaire interviews and HIV antibody tests,through recruitments of snowball sampling at a fix site and internet.Results 8.2% out of 330 MSM interviewed were diagnosed as HIV infection.The average age was 45.4±6.9 years old.85.5% respondents had children and 79.1%lived with their wives and children.69.4% had their first sexual partners with female,95.1% had anal sex in the past year and 53.8% could use condom consistently.11.5% had experienced in group sexual intercourse.36.1% had sex with his wife in the past year with 31.9% of consistent condom use.55.6% thought that homosexual behaviors had little impact with their marriage and family life.17.3% had exposed their homosexual orientation to their wives,and 24.2% felt very depressive due to their homosexuality.The multivariate logistic regression model indicated that awareness of HIV-related knowledge,use of club drugs and condom use were significantly related to HIV infection,with ORs of 7.047（1.626-30.542）,2.881（1.129-7.348）,and 4.315（1.920-9.695）respectively.Conclusion The married MSM have high HIV prevalence.HIV-related high unprotected homosexual and heterosexual behaviors cause cross infection among different population.It is urgent to conduct comprehensive intervention by taking behavior and psychological measures.
