摘要
目的通过督导评估住院医师规范化培训关键环节,了解培训工作存在的问题和不足,为提高培训质量提出针对性的改进建议。方法采取定性与定量相结合,专家评价、住院医师评价与医院自评相结合,理论考核与实践技能考核相结合的方式,抽查临床教学查房、出科考核、临床技能操作、手写病历、轮转登记手册、理论知识和读片考试,并进行住院医师问卷调查。结果临床教学查房、出科考核、临床技能操作平均成绩分别为89.69±3.18、85.03±4.13、88.08±5.00;手写病历平均成绩为76.23±8.85;理论考试平均成绩为64.20±5.88;读片考试平均成绩为67.09±11.27;住院医师对培训管理、培训实施、教师带教、能力提升的满意度在60%~80%,对收入和住宿满意度在20%~40%。结论住院医师规范化培训内涵质量需要持续改进,建议加强培训组织管理,规范培训过程管理,强化住院医师"三基"训练,重视培训师资队伍建设,保障培训住院医师待遇。
Objective By inspecting and evaluating training, to learn existing problems and insufficiencies in the the quality of the key links in standardized resident training and put forward corresponding improvement suggestions. Methods Through qualitative and quantitative approaches combination, expert evaluation, resident evaluation and hospital self- evaluation combination, and theory examination and clinical skill operation examina- tion combination methods, check the clinical teaching ward - rounds, the department rotation examinations, the residents of clinical skill operations, the residents of hand - written medical records, the residents of rotary registra- tion handbooks theory and ECG and X -ray examinations, and questionnaire surveys among the residents. Results The average scores of the clinical teaching ward - rounds and the department rotation examinations and the residents of clinical skill operations were 89.69 ± 3.18, 85.03 ± 4.13, 88.08 ± 5.00 ; the average score of the residents hand - written medical records was 76.23 ± 8.85 ; the average score of theory examination was 64.20 ± 5.88 ; the average score of the ECG and X - ray examination was 67.09 ± 11.27. The satisfactions of training management, training implementation, clinical teaching and capacity improvement of residents were between 60% and 80% , and the satisfactions of income and accommodation of residents were between 20% and 40%. Conclusion The conno- tative quality of standardized resident training requires further continuous improvement, so we suggest to enhance the training organization and management, standardize the training process management, strengthen basic theories, basic knowledge and basic skills of residents, attach great importance to clinical teaching staff construction, and guarantee income and welfare of residents.
