摘要
目的探讨对晚期肿瘤患者开展舒缓疗护的效果和意义。方法对55例晚期肿瘤患者开展舒缓疗护,并进行生存期评估和舒缓疗护效果评估,对入院前后的症状评分进行比较。结果入院后,疼痛、失眠、纳差、恶心、腹胀、胸闷、抑郁、便秘、发热、浮肿评分分别为(2.36±1.18)、(3.54±1.93)、(4.27±1.94)、(1.57±1.81)、(2.20±2.00)、(3.43±2.56)、(3.82±2.64)、(2.34±1.85)、(2.73±1.65)、(2.41±1.63)分,明显低于入院前的(6.27±2.02)、(6.82±1.99)、(6.73±2.23)、(4.07±2.99)、(3.75±3.07)、(5.07±3.50)、(5.52±3.60)、(5.41±3.10)、(5.00±2.18)、(4.30±2.36)分,差异有统计学意义(P<0.05)。55例患者中,52例在院死亡,3例出院;其中1例出院后15 d在家死亡,2例2个月后在三甲医院死亡。52例在院死亡的患者均平稳地度过临终期。患者及家属对舒缓疗护工作的满意度为98.2%。结论对晚期肿瘤患者开展舒缓疗护,能够减轻患者临床不适症状,提高患者的生命质量及患者和家属的满意度。
Objective To discuss the effect and signifiance of palliative care for patients with advanced cancer. Methods A total of 55 patients with advanced cancer were treated with palliative care, and their survival period and the effect of palliative care were evaluated. The symptom scores before and after admission were compared. Results After admission, patients had pain, insomnia, anorexia, nausea, abdominal distension, chest tightness, depression, constipation, fever, edema score respectively as（2.36±1.18）,（3.54±1.93）,（4.27±1.94）,（1.57±1.81）,（2.20±2.00）,（3.43±2.56）,（3.82±2.64）,（2.34±1.85）,（2.73±1.65） and（2.41±1.63） points, which were obviously lower than（6.27±2.02）,（6.82±1.99）,（6.73±2.23）,（4.07±2.99）,（3.75±3.07）,（5.07±3.50）,（5.52±3.60）,（5.41±3.10）,（5.00±2.18）,（4.30±2.36） points before admission, and their difference was statistically significant（P〈0.05）. Of the 55 patients, 52 died in the hospital and 3 were discharged; 1 of them died of 15 d at home after discharge, and 2 died in the third grade hospital 2 months later. 52 patients who died in the hospital passed the dying period smoothly. The satisfaction degree of patients and their family members to palliative care was 98.2%. Conclusion Palliative care for patients with advanced cancer can reduce the clinical symptoms, improve their quality of life, and the satisfaction of patients and their families.
