摘要
目的 探讨助产士实践方案对肥胖孕妇妊娠结局的影响.方法 选取2014年1月—2016年1月在四川省崇州市人民医院接受产检并进行分娩且BMI≥28 kg/m2的284名单胎初产妇为研究对象.按照随机数字表法将284名肥胖孕妇分为干预组和对照组,每组各142名.对照组接受常规产检及孕期健康教育,干预组除接受常规产检以及健康教育外,采用助产士实践方案进行全程化管理.比较两组孕妇的母体指标以及新生儿指标.结果 干预组孕妇妊娠期糖尿病发病率为8.45％（12/142）、子痫前期发病率为9.86％（14/142）;对照组孕妇妊娠期糖尿病发病率为19.72％（28/142）、子痫前期发病率为22.54％（32/142）,干预组妊娠期糖尿病发病率、子痫前期发病率均低于对照组,差异具有统计学意义（P〈0.05）.干预组新生儿低血糖发生率为10.56％（15/142）,对照组新生儿低血糖发生率为23.94％（34/142）,干预组新生儿低血糖发生率低于对照组,差异具有统计学意义（P〈0.05）.结论 助产士实践方案能够显著降低肥胖孕妇相关妊娠并发症的发生率,有效地改善肥胖孕妇不良妊娠结局.
Objective To explore the effects of midwife practice program on the pregnancy outcome of obese pregnant women.Methods A total of 284 singleton primiparas with body mass index （BMI） ≥28 kg/m2 underwent production inspection and delivery in Chongzhou People＇s Hospital from January 2014 to January 2016 were selected and divided into the intervention and control groups according to the random number table method, 142 cases respectively. Primiparas in the control group received routine health examination and health education, while primiparas in the intervention group received additional midwife practice program. The maternal and neonatal indexes were compared between the two groups.Results The incidence rates of gestational diabetes mellitus and preeclampsia in the intervention group were 8.45％ （12/142） and 9.86％ （14/142）. The incidences of gestational diabetes mellitus and preeclampsia in the control group were 19.72％ （28/142） and 22.54％ （32/142）. The incidence rates of gestational diabetes mellitus and preeclampsia in the intervention group were significantly lower than those in the control group （P〈0.05）. The incidence of neonatal hypoglycemia was 10.56％ （15/142） in the intervention group, which was significantly lower than 23.94％ （34/142） in the control group （P〈0.05）. Conclusions Midwife practice program can significantly reduce the incidence of pregnancy complications so as to effectively improve the adverse outcome of obese pregnant women.
