摘要
目的探究低强度脉冲超声(LIPUS)预防大鼠激素性股骨头坏死(SANFH)的效果。方法选择雄性SD大鼠30只,鼠龄10周,体质量300~320 g。随机分为正常组(A组)、模型组(B组)、LIPUS干预组(C组),每组10只。B组、C组腹腔注射脂多糖(20μg/kg)2次,1次/天;随后两侧臀肌交替注射甲强龙(40 mg/kg)3次,1次/天。C组造模前1周对大鼠进行双侧股骨头LIPUS干预;A组相同时间腹腔及臀肌注射等比例0.9%氯化钠溶液。4个月后处死3组大鼠并取双侧股骨标本,行股骨头大体标本观察、X射线平片检测,Micro-CT扫描观察股骨头微观结构变化并进行骨计量学分析,苏木精-伊红(HE)染色观察股骨头组织病理学改变。结果与A组比较,B组大鼠股骨头骨皮质变薄,骨小梁稀疏,于软骨下区骨小梁稀疏、断裂尤为显著,其内可见空洞样无骨小梁区,并且股骨头感兴趣区骨表面积与骨体积比值(Bs/Bv)、骨体积与总体积比值(Bv/Tv)、骨小梁厚度(Tb.Th)、骨小梁数量(Tb.N)及骨矿物质密度(BMD)等骨计量学参数明显降低,而骨小梁间隙(Tb.Sp)显著升高;HE染色病理切片显示大量空骨陷窝及骨细胞核固缩,骨小梁变窄或断裂。与B组比较,C组感兴趣区Bs/Bv、Bv/Tv、Tb.Th、Tb.N、BMD等骨计量学参数明显升高,Tb.Sp降低;HE染色病理切片显示空骨陷窝及骨细胞核固缩减少。结论 LIPUS干预组能明显改善SANFH大鼠股骨头影像学和组织病理学改变,这可能为以后临床长期服用激素的患者提供一种安全、有效预防SANFH的手段。
Objective To investigate the prevention effect of low intensity pulsed ultrasound(LIPUS) on steroid-induced avascular necrosis of femoral head(SANFH) in rats. Methods Thirty male SD rats aged 10-week with body weight of 300-320 g were randomly divided into 3 groups(n = 3): normal group(group A), model group(group B) and LIPUS group(group C). Group B and group Cwere treated with intraperitoneal injection of lipopolysaccharide(LPS)(20 μg/kg) for 2 times once a day, and then both sides of alternating injection of gluteal muscles methylprednisolone(40 mg/kg) for 3 times once a day. At the same time, rats in group A were treated with abdominal and gluteal muscle injection of 0.9 % sodium chloride solution saline. After 4-month, all of rats were sacrificed and bilateral femoral heads were obtained to test by general specimen observation, X-ray, Micro-CT scan and hematoxylin-eosin(HE) staining to acquire microstructure change, bone measurement analysis and histopathological changes. Results Compared with group A, femoral cortex of group B rats turned thinner, trabeculae became sparse, especially in subchondral bone region, and the fracture was particularly significant. Meanwhile, femoral head showed no cavernous trabecular bone area, and in region of interest bone, bone surfale/bone volume(Bs/Bv), bone volume/total volumn(Bv/Tv), trabecular thickness(Tb.Th), trabecular number(Tb.N) and bone mineral density(BMD) decreased obviously, while trabeculae speration(Tb.Sp)was significantly increased. HE staining pathology showed large number of empty bone lacuna with narrowing bone trabecula or fracture. Compared with group B, Bs/Bv, Bv/Tv, Tb.Th, Tb.N, BMD of group Cwere significantly increased, while Tb.Sp decreased; HE staining pathology showed empty bone lacuna with narrowing bone trabecula or fracture decreased. Conclusion It is demonstrated that LIPUS intervention can obviously improve the SANFH rat femoral head imaging and histopathological changes, which
