摘要
目的探讨肺结核患者外周血CD26^+CD4^+T细胞百分含量和血清中sCD26水平变化。方法采取30例健康体检者(HD),30例非耐药初治肺结核患者(S-TB)及30例多耐药结核病患者(MDR-TB)2 ml肝素抗凝外周静脉血,经CD3、CD4、CD26流式抗体标记后,Beckman流式细胞仪(FCM)计数各组CD26^+CD4^+T细胞百分比含量,ELISA检测外周血清中sCD26水平。结果 MDR-TB组外周血CD26^+CD4^+T细胞占CD4^+T细胞总数的(13.63±3.85)%,低于S-TB组(20.17±5.94)%以及对照HD组(26.55±5.77)%,比较差异有统计学意义(F=45.037,P=0.000);MDR-TB组血清sCD26含量为(617.99±55.57)μg/ml,高于健康人群对照HD组含量(410.23±36.21)μg/ml以及S-TB组含量(435.22±38.34)μg/ml,比较差异有统计学意义(F=197.287,P<0.05)。Spearman相关分析,正常人群、S-TB和MDR-TB肺结核病患者CD26^+CD4^+T细胞百分比含量与血清sCD26水平变化之间存在负相关(r=-0.391,P=0.002)。结论随着肺结核由非耐药往耐药结核病发展,外周血中CD26^+CD4^+T细胞百分含量逐渐下降,耐药肺结核病人血清中sCD26水平含量明显上调,二者成负相关,通过检测肺结核患者外周血CD26^+CD4^+T细胞百分含量与血清中sCD26水平能了解肺结核疾病的进程。
Objective To investigate the changes of percentage of peripheral blood CD26+CD4+T cell and the serum level of soluble CD26 ( sCD26 ) in patients with pulmonary tuberculosis. Methods Thirty healthy donors ( HD ), 30 patients with sus- ceptible primary tuberculosis ( S-TB ) and 30 patients with multidrug-resistant tuberculosis ( MDR-TB ) were enrolled in this stud- y. 2 ml peripheral venous blood with heparin anticoagulation was stained with muhicolor labeled monoclonal antibody of CD3, CD4 and CD26. Beckman flow cytometry ( FCM) was used to count the percentage of CD26+CD4+T cell. ELISA was employed to deter- mine peripheral serum level of sCD26. Results The percentage of CD26+ CD4+ T cell in all CD4+ T cells was significantly lower in the MDR-TB group ( ( 13.63±3.85)%) than in the S-TB group ((20.17±5.94)%) and HD group ((26.55±5.77)%) (F= 45.037, P=0.000). The serum level of sCD26 was significantly higher in the MDR-TB group ( (617.99±55.57) ug/ml/ than in the HD group ((410.23±36.21) ug/ml) and S-TB group ((435.22±38.34) ug/ml) (F= 197.287, P=0.000). Spearman corre- lation analysis showed that there was a negative correlation between the percentage of CD26+ CD4+ T cell and the serum level of sCD26 in healthy donors, patients with susceptible primary tuberculosis and patients with multidrug-resistant tuberculosis ( r= -0.391, P= 0.002). Conclusions With the development of puhnonary tuberculosis from susceptible primary tuberculosis to multidrug-resistant tuberculosis, the percentage of peripheral blood CD26+CD4+T eell decreases gradually, while the serum level of sCD26 increases significantly, and there is a negative correlation between them. Determination of the percentage of peripheral blood CD26+ CD4+ T cell and serum level of sCD26 in patients with pulmonary tuberculosis is conducive to knowing the progression of tuberculosis disease.
