712例肺结核合并糖尿病患者临床疗效分析 被引量:1

摘要 目的 探讨肺结核合并糖尿病患者的临床特征及疗效。方法 对2015年1月-2018年12月在本院住院治疗的肺结核合并糖尿病患者712例,按照降糖方式不同分为胰岛素组393例及口服降糖药组319例,对比两组患者的抗结核治疗效果。结果 两组患者在性别、年龄、文化程度、职业分布上均无统计学差异(P>0.05);胰岛素组治疗前血红蛋白水平及白蛋白水平高于口服降糖药组(血红蛋白:χ^(2)=-4.129,P<0.001;白蛋白:χ^(2)=-13.905,P<0.001);两组患者在治疗前的临床特征分布上基本一致;胰岛素组在治疗3个月末、6个月末的痰菌转阴率、空洞闭合率、病灶吸收率上明显高于口服降糖药组(P<0.05);胰岛素组在抗结核治疗前后空腹血糖、餐后2h血糖水平明显低于口服降糖药组(P<0.05)。结论 控制血糖是治疗肺结核合并糖尿病的关键,抗结核治疗中及早应用胰岛素效果更佳。 Objective To explore the clinical characteristics and curative effect of tuberculosis with diabetes.Methods From January 2015 to December 2018,712 patients with pulmonary tuberculosis and diabetes in our hospital were divided into two groups:393 patients in insulin group and 319 patients in oral hypoglycemic drug group according to the different ways of reducing blood sugar.The antituberculosis effects of the two groups were compared.Results There was no significant difference in gender,age,education level and occupation distribution between the two groups(P>0.05);the hemoglobin level and albumin level in the insulin group before treatment were higher than those in the oral hypoglycemic group(hemoglobin:χ^(2)=-4.129,P<0.001;albumin:χ^(2)=-13.905,P<0.001);the distribution of clinical characteristics of the two groups before treatment was basically the same;insulin At the end of 3 and 6 months,the sputum negative rate,cavity closure rate and focus absorption rate in the insulin group were significantly higher than those in the oral hypoglycemic group(P<0.05);the fasting blood glucose and 2-hour postprandial blood glucose level in the insulin group were significantly lower than those in the oral hypoglycemic group(P<0.05).Conclusion Controlling blood glucose is the key to the treatment of pulmonary tuberculosis with diabetes mellitus,and the effect of early application of insulin in the treatment of anti-tuberculosis is better.
