p73基因多态性与原发性肝癌易感性的研究 被引量:3

Study on p73 gene polymorphism and genetic susceptibility to hepatocellular cancer

摘要 目的 研究p73基因多态性与原发性肝癌的遗传易感性的关系.方法 选择我院原发性肝癌患者100例,相同地区,年龄、性别与肝癌(HCC)配对的健康体检者100名.采用基质辅助激光解吸电离飞行时间质谱法(MALDI-TOF)及DNA 测序法,检测p73基因G4C14-A4T14位点的多态性,用χ^2检验比较基因型在肝癌组和对照组间的差异,Logistic 回归分析计算比值比(Odds ratios,OR)和 95%可信区间(Confidence Intervals,CI),并以性别和年龄进行校正,对基因多态性和肝癌的易感性进行关联分析.采用 SPSS 18.0 软件进行统计分析.结果(1)HCC组和对照组在年龄(χ^2=0.185,P=0.667)、性别(χ^2=0.026,P=0.873)、饮酒史(χ^2=0.427,P=0.514)、癌症家族史(χ^2=0.058,P=0.809)组成上差异均无统计学意义;(2)p73 rs1801173位点共检测到CC、CT、TT三种基因型,其分布频率在在对照组,GC/GC、GC/AT,AT/AT基因型的频率分别为65.0%,28.2%,6.8 %,在HCC病例组分别为62.2%,31.1%,6.7 %,两组比较差异无统计学意义(χ^2=0.326,P=0.568).(3)经年龄、性别、吸烟状况、饮酒情况、HBV携带状态和家族癌症史分层分析后发现,P73基因GC/AT + AT/AT基因型明显增加HBsAg(+)患者及女性[HBsAg(-)]肝癌的发病风险,HBsAg(+)(χ^2=3.916,P=0.048)及性别为女性(χ^2=6.545,P=0.01)为独立的危险因素,而与年龄、吸烟、饮酒、肿瘤家族史无明显相关.结论 (1)p73 G4C14-A4T14的变异基因型(GC/AT + AT/AT)明显增加HbsAg阳性患者肝癌的风险;(2)p73 G4C14-A4T14的变异基因型(GC/AT + AT/AT)增加女性患者肝癌的风险. Objective To evaluate the potential role of the separately p73 gene polymorphism on hepatocellular carcinoma(HCC) susceptibility.Methods A hospital-based case-control study was conducted in 100 cases HCC patients and 100 cases age,sex-matched cancer-free controls in the same region.The matrix-assisted laser desorption ionization time-of-flight mass spectrometry and DNA sequencing methods were used to analyze polymorphisms of p73 genetic polymorphisms (G4C14-A4T14).Using χ^2 test to exam the differences of genotype between the HCC group and control group,logistic regression was used to analysis odds ratio(Oddsratios,OR) and 95%CI(Confidence Intervals,CI),adjusted for age and sex,analysis the association between SNPs of P73 polymorphisms and HCC.All the results were analyzed by SPSS 18.0.Results (1)The differences of age(χ^2=0.185,P=0.667),gender(χ^2=0.026,P=0.873),drinking status(χ^2=0.427,P=0.514),and family history (χ^2=0.058,P=0.809)of cancer composition between HCC group and control group were not statistically significant.(2)There were CC,CT and TT three genotypes in p73 rs1801173 C/T.The frequencies of CC,CT and TT genotypes of P73 C/T in controls and HCC cases were 65.0%,28.2%,6.8 % and 62.2%,31.1%,6.7%,respectively,and there was no significant difference between the two groups(χ^2=0.326,P=0.568).(3)When stratifying by age,gender,smoking status,alcohol consumption,HBV carrier status and family history of cancer we found that the variant genotypes of GC/AT + AT/AT in P73 was associated with a significantly increased risk of HCC among HbsAg-positive individuals(χ^2=3.916,P=0.048) and female(χ^2=6.545,P=0.01),and there were no significant correlation with age,smoking,alcohol consumption,family history of cancer.Conclusion (1)p73 G4C14-to-A4T14 dinucleotide polymorphism may play a role in the development of chronic HBV-infected HCC in the Chinese population.(2)p73 G4C14-to-A4T14 dinucleotide polymorphism may play a role in the development
