摘要
目的:研究2型糖尿病(T2DM)患者血清CTRP1的水平变化,探讨CTRP1在T2DM发病中的作用及其与糖脂代谢、胰岛素抵抗的关系。方法:随机选取2015-11~2016-08在佳木斯大学附属第一医院内分科就诊的60例初诊T2DM患者,同期30例健康体检者为对照组,记录血压、身高、体重等一般临床指标,检测血清空腹血糖(FPG)、糖化血红蛋白(Hb A1c)、甘油三酯(TG)、胆固醇(TC)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、空腹胰岛素(FINS)水平,计算体质量指数(BMI)、腰臀比(WHR)、胰岛素抵抗指数(HOMA-IR),采用ELISA法检测CTRP1的水平。结果:初诊T2DM患者血清CTRP1水平明显高于正常对照组[284.77±51.22)vs(124.29±41.34),P<0.05]。血清CTRP1水平与BMI、WC、WHR、TG、TC、LDL、FPG、Hb A1c、FINS、HOMA-IR正相关,与HDL负相关(P<0.05)。多元回归分析显示BMI、HOMA-IR、Hb A1c是血清CTRP1的独立影响因素。结论:初诊T2DM患者血清CTRP1水平升高,CTRP1与糖脂代谢紊乱及胰岛素抵抗密切相关,可能参与了2型糖尿病及胰岛素抵抗的病理过程。
Objective : To investigate the serum level of C 1 q/TNF - related protein (CTRP) 1 in type 2 diabetes mellitus (T2DM) subjects, and the role of CTRP1 on the pathogenesis of T2DM and the association of plasma CTRP1 level with glucose and lipid metabolism and insulin resistance. Methods:A total of 60 T2DM patients and 30 normal controls were recruited. General parameters such as blood pressure, height, weight and some other parameters were measured, including fasting plasma glucose (FPG), glycosylated hemoglobin (HbA1 c), erum tri- glyceride (TG), cholesterol (TC), high density lipoprotein (HDL), low density lipoprotein (LDL), fasting insu- lin ( FINS), then calculate body mass index ( BMI), waist - to - hip ratio ( WHR), insulin resistance index ( HO- MA -IR). Serum CTRP1 levels Were determined by ELISA kits in all subjects. Results: Serum CTRP1 levels were significantly higher than the normal control group [ ( 284.77 ± 51.22 ) vs ( 124.29 ± 41.34), P 〈 0.05 ]. Furthermore, Pearson correlation analysis showed that CTRP1 levels were positively correlated with BMI, WC, WHR, TG, TC, LDL, FBG, HbA1 c, FINS, HOMA - IR and negatively correlated with HDL ( P 〈 0.05 ). Multiple linear regression analysis showed that BMI, HOMA -IR, HbAlc were independent relevant factors for CTRP1. Conclusion :The plasma level of CTRP1 is significantly higher in T2DM patients than normal subjects, and it is closely correlated with glycolipid metabolism disorders and insulin resistance. CTRP1 may contribute to the pathophysiology of T2DM and insulin resistance.
