血浆脂肪细胞因子与冠心病及糖尿病的相关性研究 被引量:16

Correlation of plasma adipokines with coronary heart disease and diabetic mellitus

摘要 目的探讨血浆脂肪细胞因子C1q/肿瘤坏死因子相关蛋白-9(C1q/TNF-related protein-9,CTRP-9)、脂联素(adiponectin,APN)及肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)与冠心病及糖尿病的相关性。方法行冠状动脉造影患者88例,其中单纯冠心病组39例(单纯CHD组)、冠心病合并糖尿病组28例(合并组)、非冠心病及非糖尿病患者21例(对照组),3组均行血生化检测,采用ELISA法检测血浆CTRP-9、APN及TNF-α水平,分析冠心病及糖尿病与相关因素的关系。结果单纯CHD组吸烟比例、低密度脂蛋白胆固醇(low density lipoprotein-cholesterol,LDL-C)及TNF-α水平高于对照组,高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)、CTRP-9及APN水平低于对照组(P<0.05);合并组血浆TNF-α水平高于单纯CHD组,CTRP-9、APN低于单纯CHD组(P<0.05);合并组血浆LDL-C、TNF-α水平高于对照组,HDL-C、CTRP-9及APN水平低于对照组(P<0.05);年龄(r=0.264,P=0.013)、糖尿病(r=0.394,P=0.000)、TNF-α(r=0.333,P=0.002)及LDL-C(r=0.330,P=0.002)与冠心病呈正相关,CTRP-9(r=-0.383,P=0.000)和APN(r=-0.435,P=0.000)及HDL-C(r=-0.239,P=0.025)与冠心病呈负相关;体质量指数(r=0.233,P=0.029)及TNF-α(r=0.425,P=0.000)与糖尿病呈正相关,CTRP-9(r=-0.350,P=0.001)和APN(r=-0.486,P=0.000)与糖尿病呈负相关;CTRP-9与APN呈正相关(r=0.312,P=0.003),TNF-α与CTRP-9(r=-0.298,P=0.005)及APN(r=-0.324,P=0.002)呈负相关。结论冠心病及糖尿病均与CTRP-9及APN呈负相关,与TNF-α呈正相关;CTRP-9和APN对冠心病及糖尿病有保护作用,TNF-α作用相反。 Objective To investigate the correlation of plasma Clq/tumor necrosis factor-related protein-9 (CTRP-9), adiponectin (APN) and tumor necrosis factor-α (TNF-α) with coronary heart disease (CHD) and diabetic mellitus (DM). Methods Eighty-eight patients undergoing coronary angiography were divided into three groups: CHD group (n= 39), CHD+DM group (n=28), and control group (with no CHD or DM, n=21). The biochemical parameters and the levels of CTRP-9, APN and TNF-α were detected by ELISA technique in three groups. The correlations of CHD and DM with the related factors were analyzed. Results The proportion of smoking, the level of low density lipoprotein-cholesterol (LDL-C) and TNF-α were significantly higher, and the levels of high density lipoprotein-cholesterol (HDL-C), CTRP-9 and APN were lower in CHD group than those in control group (P〈0. 05). The level of plasma TNF-α was higher, and the levels of CTRP-9 and APN were lower in CHD+DM group than those in CHD group (P〈0.05). The levels of LDL- C and TNF-α were higher, and the levels of HDL-C, CTRP-9 and APN were lower in CHD-4-DM group than those in control group (P〈0.05). CHD was positively correlated with age (r=0. 264, P=0. 013), DM (r=0. 394, P=0. 000), TNF-α(r=0. 333, P=0. 002) and LDL-C (r=0. 330, P=0. 002), and negatively correlated with CTRP-9 (r=-0. 383, P=0. 000), APN(r= -0. 435, P=0. 000) and HDL-C (r= -0. 239, P=0. 025). DM was positively correlated with body mass index (r= 0. 233, P= 0. 029) and TNF-α (r= 0. 425, P = 0. 000), and negatively correlated with CTRP-9 (r=-0.350, P=0.001) and APN (r=-0.486, P=0.000). CTRP-9 was positively correlated withAPN (r=0.312, P=0.003). TNF-α was negatively correlated with CTRP-9 (r=-0. 298, P=0.005) and APN (r=-0. 324, P=0. 002). Conclusions CHD and DM are negatively correlated with CTRP-9 and APN, and positively correlated with TNF-α. CTRP-9 and APN have a protective effect on CHD and
