肺癌患者血清和支气管灌洗液中高迁移率族蛋白-1水平改变的临床意义 被引量:2

The effect of HMGB-1 in serum and bronchial lavage fluids in patients of lung cancer

摘要 目的分析肺癌患者血清和支气管灌洗液中高迁移率族蛋白-1(HMGB-1)水平改变的临床意义。方法收集2014年4月—2016年3月山东省日照市人民医院肿瘤科就诊的肺癌患者94例(肺癌组)以及肺部良性病变患者80例(良性肺病组)作为研究对象。分析比较血清和支气管灌洗液中HMGB-1浓度与肺癌分期、淋巴结和远处组织转移的相关性。结果肺癌组血清及支气管灌洗液内HMGB-1水平均高于良性肺病组[血清:(5.4±0.4)ng/ml vs.(1.4±0.3)ng/rnl;支气管灌洗液:(12.4±2.5)ng/ml vs.(2.4±0.5)ng/ml,t=2.055、2.831,P=0.024、0.004],肺癌患者支气管灌洗液中HMGB-1浓度高于血清(P<0.05)。肺癌组血清及支气管灌洗液HMGB-1浓度比较:TNM分期Ⅰ~Ⅱ期低于Ⅲ~Ⅳ期(P<0.05),有淋巴结转移高于无淋巴结转移(P<0.05),有远处转移高于无远处转移(P<0.05)。血清和支气管灌洗液HMGB-1水平与肺癌呈正相关(血清:r=0.705,P=0.009;支气管灌洗液:r=0.846,P=0.001)。支气管灌洗液HMGB-1水平对于诊断肺癌(AUC=0.828,cut-off值=14.2 ng/m1)、TMN分期(Ⅲ~Ⅳ期)(AUC=0.778,cut-off值=18.3 ng/m1)、淋巴结转移(AUC=0.756,cut-off值=19.8 ng/ml)、远处转移(AUC=0.712,cut-ofr值=21.2 ng/ml)均有价值。结论肺癌患者血清和支气管灌洗液中HMGB-1水平明显上升,有助于肺癌的早期诊断和分期预测。 Objective To evaluate the correlation value between the level of serum HMCB-1 and bronchial lavage fluids and cancer metastasis in patients of lung cancer.Methods Ninety four lung cancer patients and eighty pneumonia benign disease patients were enroll in this study.The contents of serum HMCB-1 and bronchial lavage fluids were measured and recorded.Correlation analysis was used to estimate the relation between serum HMCB-1 and tumor stage.Results The level of serum HMCB-1 of and bronchial lavage fluids in lung cancer group were significantly higher than those in control group[serum:(5.4±0.4)ng/ml vs.(1.4 ±0.3)ng/ml,t =2.055,P =0.024;BLF:(12.4 ±2.5)ng/ml vs.(2.4 ±0.5)ng/ml,t =2.831,P =0.004].The contents of serum HMGB-1 were higher than those of bronchial lavage fluids in lung cancer patients[(5.4±0.4)ng/ml vs.(12.4 ±2.5)ng/ml,t =2.327,P =0.013].Compared with Stage Ⅲ-Ⅳ patients,the HMGB-1 levels of both serum and bronchial lavage fluids in Ⅰ-Ⅱ patients were increased[serum:(4.2 ±0.3)ng/ml vs.(6.9±0.8)ng/ml,t =2.430,P =0.010;BLF:(9.4 ± 1.9)ng/ml vs.(14.7 ±3.5)ng/ml,t =2.565,P =0.008].The lung cancer patients with lymphatic metastasis had higher levels of serum HMCB-1 and bronchial lavage fluids compared with those without lymphatic metastasis.ROC analysis showed that the AUC for lung cancer diagnosis of serum and bronchial lavage fluids of HMCB-1 were 0.722 and 0.828 respectively.Correlation analysis showed that both serum and bronchial lavage fluids of HMGB-1 were positively related with lung cancer.Conclusion The levels of serum and bronchial lavage fluids HMGB-1are elevated in lung cancer patients and used for early detection and stage prognosis.
